STEP 1: Day 1 >Initial Visit for review medical history to get Orders for Labs, Vaccinatioms & X-ray
Make an Appointment and come to our Milpitas or San Jose locations. You can make group appointments for your family. Same-day or next-day appointments are usually available. First Visit can be in-person in our locations or a virtual video chat. One adult applicant in the family need to see us to go over vaccinations, medical history and explain you process and give you orders for labs, vaccinations and X-ray. However, in second or final visit all applicants must appear before one of our Civil Surgeons team for physical exam, sign I-693 in civil surgeon presence and pick up your completed i-693 sealed envelop.
Here is detailed guidance for your 1st Visit.
Bring any existing Vaccination Records and a photo ID. Do not worry, if you do not have vaccination records.
Download the latest I-693 Form (ver 1/20/25, expiring 9/30/27) & type in it or fill in your demographic info on the first 2 pages in Black ballpoint pen. One form for each applicant is required. Print all 14 pages single-sided. Read FAQs, cost, and form-filling instructions. Do not sign or date your I-693. You need to sign it in the presence of the civil surgeon.
Get your Immigration Medical Exam done. We will assess the Labs required & gaps in Vaccinations. You will be given orders for Labs, Vaccinations & chest X-rays if needed. We have a volume discount negotiated with the labs to save you money.

Step 2> Day 1: Go to Lab to give Sample
Take our Prepaid cost savings Lab Order and go to either LabCorp, Quest, or IHD Lab location near you or us. No fasting is required. We will give you the lab's location maps. Alternatively, you may arrange your lab yourself & take unpaid Lab orders, and pay the lab directly. Lab results take usually 4 to 5 working days.
At the lab's portals, you can make Appointment and View results online. For Quest at MyQuestDiagnostics.com , LabCorp at labcorp.com/patients and IHD at Ihdlab.com/tests-for-individuals. Or call us or emails us if you need assistance. . Or just call or email us for any assistance.
You can interpret your lab reports as follows:
For the QF TB Test, RPR Syphilis Test, and Gonorrhea Test, the results should be Negative or Non-Reactive (GREEN Color)
If the QF TB Blood Test is Positive (RED Color), an X-ray is required to rule out Active TB. Refer to the X-Ray Orders section for recommended action & X-ray facilities. TB Test comes positive in about 10% of population except persons born Europe.
For immunity to tests; results values will show if you are immune or not and will require vaccination if no or low immunity.
Get your missing Vaccinations as per the order given on 1st visit and or if you do not have immunity per lab results. You can take all the vaccines on the same day. Our Civil Surgeon & staff will guide you on what, when & where you can get your vaccinations with or without insurance at no or low cost.
Go to any Pharmacy (Walgreens, Costco, Walmart, Safeway, CVS, etc.) or your provider of your choice and just bring proof of vaccination with you on your 2nd visit.
Insurances cover Vaccination costs under preventive care. If you do not have insurance, we will guide you during your visit to get vaccinations at low or no cost.
If multiple doses of a vaccine are required at certain intervals, You do not have to wait to complete the series. Just take the first shot of the vaccine and your I-693 will be processed with a note of insufficient interval indicating that additional shots have been scheduled by the applicant.
COVID-19 Vaccine: ALERT: As of 1/22/2025 USCIS has Waived COVID-19 Vaccination Requirement for Adjustment of Status Applicants (I-693).
STEP 4: (Last Step) Day 5 : >Pick up your I-693 Form
Make a follow-up 2nd appointment online. Allow about one hour for each applicant for your 2nd visit. See detailed 2nd visit instructions.
On the day of the appointment, bring your printed I-693 form per instructions in case you did not bring it on the 1st visit, and your driver's license or passport or state photo ID. Do not sign your I-693, you have to do it before the civil surgeon. The download links for forms and instructions are given in the section below on this page. The latest I-693 Form (ver 1/20/25 expiring 9/30/2027) can be downloaded from our website.
If you did not select the prepaid lab option, print your Lab Reports and bring them with you. If you selected pre-paid orders, lab reports come to us directly in EHR. Print & bring your Vaccination records. Do not show us your reports on your phones.
One of our Civil Surgeons in our group practice will complete your I-693 form with your Lab, Vaccination & X-ray reports. You will sign the form in the presence of Civil Surgeon & show your photo ID. All applicants must appear before Civil Surgeon for physical exam and sign.
You will be given your sealed I-693 Envelope for USCIS submission & an electronic copy for your records. Your i-693 compleetd form can be submitted anytime and expires on the expiry date specified on I-693 form (current ver exires 09/2027) or as may deem necessary per USCIS.
Please note that your initial visit can be in-person in our locations or a virtual video chat. We accept any vaccination records from any country. You can take our orders for Vaccinations to any pharmacy and will be covered by your insurance. If you have no insurance, we will guide you to get the Vaccinations at low or no cost. We send you to independent labs & X-ray places that offer a 60% volume discount with our orders. We offer concierge, connected, & cost-effective quality medical exams.
Normal process time takes about 5 to 7 working days. We can also expedite the process, at an extra cost.

ALERTS:
The latest version of the i-693 form Edition 01/20/25 (expiring 09/30/27) has been released on 9/11/25. The latest i-693 form and instructions can be downloaded from our website in Forms menu.
Laboratory Testing: Effective 9/1/2025; Civil surgeons are permitted to order laboratory testing up to two weeks prior to the visit with the civil surgeon. The labs must be ordered by the civil surgeon, and or the civil surgeon should be able to directly access the lab results. Hand-carried results cannot be accepted unless they can be independently verified with the laboratory.
Note: We are connected with Quest, LabCorp, Fondation and IHD Labs to both order and receive lab results, which are directly reported into our EHR. To ensure chain of custody, prevent tampering, and maintain USCIS compliance, lab from Kaiser, Stanford, Sutter, or other labs must send results directly to the civil surgeon via fax, secure email, or postal mail. Hand-carried results cannot be accepted.

On your first visit, bring with you any existing Vaccination Records (no worry, if you do not have them), a driver's license or passport, and an I-693 printed Form. We charge $200 for the i-693 immigration medical exam & $210 for processing & preparation of the I-693 form. USCIS required Lab tests can cost in the range of $0 to $200 depending upon the applicant's age & medical history. We refer you to outside labs such as Quest, LabCorp or IHD Lab for cost savings & convenience. ALERTS:
Effective 1/22/2025 USCIS has waived all COVID-19 Vaccination Requirement for Adjustment of Status Applicants (I-693).
Effective April 4, 2024, any Form I-693 that was properly completed and signed by a civil surgeon can be used indefinitely as evidence to show that the applicant is not inadmissible on health-related grounds. However, the I-693 form expiry date will determine if a new form is required.
You are advised to Download & use the latest fillable i-693 Form from our website. Our clinic address is also pre-populated in these i-693 forms edition 01/20/25 expiring 09/30/27. This new I-693 form was released on 9/11/25.
The i-693 form should be filled & printed using Adobe Reader. Do not use Chrome, Edge, or Safari Browsers to fill pdf. Adobe Reader will auto-fill your name and A number on top of each page and ensure the proper formatting & printing of forms and barcodes on each page. Print all 14 pages, single sided in black ink and bring it with you preferably on your first visit but is required by 2nd visit to pick up your completed I-693 form.

1. It takes 2 visits: To complete the i-693 Medical Exam, Lab Tests, Vaccinations, etc., you'll need to make 2 visits and allow for 5 working days. Lab testing typically takes 4 to 5 working days. However, if time is a factor, we may be able to expedite the process and get everything done in one visit. 2. During your First visit; an I-693 immigration-focused medical exam will be done. Our provider will take your medical history, assess any required vaccination gaps, and give you lab & vaccination orders. You can take these orders to any place or a place referred by us to get vaccinations or labs done.
If you do not have vaccination records, do not worry. Most of the applicants from other than European countries do not have records. You can get an immunity test showing immunity from natural diseases or vaccinations.
For your first visit, please bring the following: vaccination records, a photo ID such as a valid Driver's License or a Passport & the I-693 form filled with your applicant information on the first 2 pages and filled on top of each page with your name & A-number.
3. Download and fill out Your I-693 Form with your demographics: You are strongly advised to read about the I-693 medical exam & read FAQs and More Info on our website for detailed instructions. You can download the I-693 forms here.

Brief Form Filling Instructions: You must fill the first and second pages of I-693 with your demographics and the top of each page with your name and A-number(if you have it). You can either fill it with BLACK ballpoint with Capital (upper case) Letters or you can type into it using your computer. Use Adobe PDF Reader to fill and print the I-693 form for the Applicant Info section and print single page I-693 without any printer scaling on regular weight (20 lb) paper. Our form is designed to be fillable and can pre-populate each page top line repetitive info of your Name and A-number provided you use Adobe PDF Reader and do not use browsers (Chrome or Edge or Safari) as the browser extensions do not support auto filling I-693 form and printing barcodes.
A typed-in form is preferred over the hand-filled page and OCR scans better by UCIS for expedient processing. Make sure the barcode is printed on all pages. If you fill i-693 by hand, fill it with BLACK ink ballpoint pen and use CAPITAL letters. Do NOT sign or date your form. Applicant has to be sign in the presence of Civil Surgeon. 6. Sealed Completed I-693 Form: After your Labs come back, at the 2nd appointment, you will get your sealed I-693 form envelope reviewed & signed by one of our Civil Surgeon in the team. You will also get an electronic copy of the completed I-693 for your records. Do not leave without it.
You must pick up your I-693 within 30 days of the medical exam and submit it to USCIS or your attorney. Make sure to mail it via FedEx, UPS, or USPS with a tracking number.
7. Cost of Medical Exam:

The USCIS Immigration Medical Exam is a medical examination that is required for certain immigrants to the United States. The exam is conducted by a USCIS-approved civil surgeon and includes a physical examination, a review of the applicant's medical history, and laboratory tests. The purpose of the exam is to determine if the applicant is inadmissible to the United States due to a medical condition or a possible threat to society. The Immigration Medical Exam differs from a general annual physical exam. Per USCIS guidelines, the medical exam focuses on communicable diseases, drug dependency, and other mental or physical conditions that can constitute an applicant to be a threat to society and USCIS inadmissibility criteria It does not focus on your lipids, blood, diabetes, kidney, etc. diseases.
The immigration medical exam is a crucial part of the green card application process. Under U.S. immigration law, all green card applicants must complete the immigration medical exam to confirm that they are in good health and are fit to live in the United States as permanent residents.

- Insurance Card, if you have Health Insurance Coverage & want to use insurance for vaccinations. An insurance card is needed for a provider to bill Insurance.

We charge $20 if you do not bring a printed form and we need to print it or fill it out. I-693 Form Filling Instructions:
- One form per applicant is required. Type and Print only in Black ink. If filling by hand, use all CAPITAL letters and use a Black ink ballpoint pen only.
- Fill the 1st page with Applicant info; Fill a few lines on page 2 asking you how USCIS can contact you. Also, each page's top line has boxes asking for your family name, first name, and A-number. Please fill these boxes on all 13 pages including the last page. If you do not have an A-number given; leave it blank. If you download the form from our website, save it in a separate folder and then open it with Adobe Reader (do not open it with your IE, Edge, or Chrome Browser). In Adobe Reader or Adobe DC; you can type into it and it will automatically fill up and populate the top line on all pages and print the barcode on the bottom. Chrome or Edge or other browsers may not support full functionality.
- DO NOT sign or put any date. After filling pdf form; print the i-693 form single or double-sided. Print it without any scaling or fit to the page but only in its original condition. Do not use heavy paper as it is difficult to fold and scan in the document scanners. Use standard 19-20 Lb. paper. Do not use heavy thick paper such as 24lb or higher thick paper. Thick paper can jam the USCIS document scanners. Do not staple.

How Long Does the Process Take?
- It will require 2 visits. The average time for the expedited process is 4 to 7 working days, depending upon your vaccination, lab results, medical history, and your timely follow-up. If you have an urgency, we will help you expedite it for you.
- On 1st Visit: a) Bring the i-693 filled form, vaccination records, Photo ID & medical history for our review. You will have a Physical Exam & our Physician will evaluate what Vaccines & tests you will need.
- We will give you order forms for Lab tests for Gonorrhea, Syphilis & TB (QuantiFeron Blood Test). Lab results can take up to 4 or 5 working days. Let us know when your Lab results come; we will make you an appointment for your second visit.
- After reviewing your medical history & age, we will determine the needed vaccines or give you orders for you to get them from anywhere you so desire. Vaccinations are covered by insurance as preventive care. We have Walgreens and CVS within a few blocks.
On Your 2nd Visit: Our USCIS Civil Surgeon will review your i-693 completed package, vaccines, and lab results and see if anything else is required per USCIS guidelines based on your info. You can download your Lab Results from the patient portal of the lab or our patient portal. You can download instructions on how to interpret your Lab Results by clicking here.
- You will be given a sealed envelope of your exam to be submitted to USCIS or your attorney. A duplicate electronic PDF copy will be provided to you via your email.
- Rarely, do you need an extra day for an X-ray of the chest, if your TB Test comes back positive.

What Vaccinations are required by USCIS?
Bring with you any of your Immunization Records to determine what will be needed per USCIS.
If you do not have vaccination records, do not worry. Most of the applicants do not have all the vaccination records. You can do an inexpensive lab test for immunity showing that you have acquired immunity either via vaccination or natural disease. If you do not have immunity, then you can take vaccinations. Do NOT take vaccine till you have consulted the civil surgeon as vaccines are dependent upon your age, medical history, contra-indications, country of origin or travelled.
a) Adults: Tdap, MMR, Varicella (Chicken Pox) & Influenza, for age > 2 years b) Children: Must be up to date with a vaccination schedule for their age per CDC guidelines for Hepatitis A and B, Influenza and influenza b (Hib), Measles, Mumps, & Rubella (MMR), Meningococcal, Pneumococcal, Pertussis (whooping cough), Polio, Rotavirus, Tetanus & Diphtheria, Varicella (chicken pox). Vaccine schedules are available at CDC websites or USCIS Vaccines website. or click on our Vaccinations page for easy-to-understand requirements. You can also consult your provider/physician for an applicable vaccine for your specific needs that depends upon your age, health history, vaccination records, country of origin, etc. For your convenience; we stock and can give all vaccines in our Clinic or you can get them anywhere such as from your healthcare provider, CVS, Walgreens, Walmart, Costco, county clinics, etc.
Please note that USCIS vaccination requirements are specific to foreign-born immigrants seeking permanent residency. These may differ from CDC guidelines for U.S.-born citizens, as well as requirements for work, school, college, or travel. USCIS Civil Surgeons must follow USCIS guidelines for the I-693 Medical Exam.

- Vaccines are required by USCIS for communicable diseases. The Physician will do their best within their discretion and USCIS guidelines based on your medical history; give you choices to either do a test for immunity or take vaccines. You can decide based on your filing timeline, cost, and the health & safety of others. Do not worry. Records are hard to get from many countries. We can guide you to the best outcome.
- For Example; MMR requires two shots 28 days apart. We can get one today & schedule your take 2nd shot for MMR later. You can file the application now & give the proof of vaccination to your attorney or take it with you on your USCIS interview and or fill it with your application.

- QuantiFeron (Tuberculosis) for all ages
- Gonorrhea for the age group 18-24. Other ages depend upon medical history.
- Syphilis for ages 18-45 years. Other ages depend upon medical history.

Lab Information All your lab tests are done by outside labs. We recommend the cheapest Lab at volume discount prices of 60% off at Quest, LabCorp or IHD labs with our orders. There are 4 labs in our area and their charges range from $400 to $1,000 for the 3 required lab tests but you will pay 1/3rd of this with the pre-paid lab orders from us. Many insurances will not cover lab charges as these are very specific to immigration exams. For convenience and the total low cost of your immigration medical exam; we have negotiated a package price with 3 labs that is significantly lower than the regular cost. The lab is accredited & accepted by USCIS and is an independently accredited facility with over 50 locations across the Bay Area for your convenience. 3 lab locations are within a few minutes away from both our Milpitas and San Jose USCIS clinics locations. You will be responsible for paying directly to the lab if you select the lab of your choice; as we are not connected in any way to labs. If we order the labs on your behalf, you will have to prepay the lab charges. We order only the labs you will need. You can download Lab results from the Lab Portal. If results are within range and normal, usually are shown in GREEN color, which means no further action is required. You can come and get your I-693 completed form. If the results are out of the normal range (abnormal), usually show in RED color, which means that further action is required. If the QF TB Test is positive (Red color), you need an X-ray to rule out active TB. If immunity tests are abnormal ( Red Color), then you need to take that vaccination. If the RPR or Gonrehea test is reactive (abnormal) usually shown in RED color, just call us for the next steps to perform confirmatory tests and discuss options.

We will guide you to get your vaccines from preferred pharmacies at 25% cheaper than us and with our civil surgeon orders, vaccines are covered 100% by your insurance. You pay ZERO $ for Vaccines. If you do not have insurance, we will guide you to get these at no or low cost.
Your Employer may Pay for Medical Exam Related Expenses or your HSA or FSA Card will Cover Expenses.

1. Download the Latest USCIS i-693 form form & instructions from our website link. Type and fill up Part I of this form. Here are a few USCIS Form Filling Tips for completing I 693.
2. Current Passport and or a US Driver's License with a photo for each applicant.
3. Any Vaccination records. If you do not have, do not worry. Most of applicants do not have complete records.
You are required to bring to your first appointment all of the documents in your possession about your immunization status. The documented evidence of your immunizations will be transferred into your I-693 form. The table of the USCIS Required Immunizations is included by Age is your guidance for your vaccination needs. Individuals without proof of immunization requirements to be vaccinated according to the USCIS medical exam requirements, before their I-693 can be signed off. You have the option to receive the vaccination either at the pharmacy such as CVS, Walgreens, Costco, Walmart, etc., the County Health Department, your private physician's office, or Kaiser or PAMF depending upon your insurance. CVS, Walmart, Costco, and Walgreens accept walk-ins. With doctor's orders, vaccines are covered by all insurance and are free of charge. If you do not have insurance, we can guide you to the lowest-cost place to get vaccinations.

About the i-693 Medical Examination
- Only civil surgeons team of designated by the USCIS Clinic, P.C. a group practice and perform immigration medical examinations.
- The doctor must sign and endorse the form before it is returned to you
- In case you have performed X-rays, you should be provided with the films once you are cleared
- Applicants that require follow-up sessions must include the supplement to Form I-693
- Form I-693 must be completed in English.
- Form I-693 is delivered to you in a sealed envelope (DO NOT OPEN)
- The applicant is responsible to pay for all fees related to the medical examination

Here are a few Tips from USCIS and our decade of experience in processing over 27,000 I-693.
Complete Your Form Accurately:
- Use the most current form version from our or USCIS website.
- When possible, download the form from our website and complete it with a computer.
- We recommend you type & fill in the form on your computer. If you handwrite your answers, use a BLACK ink ballpoint pen. Make sure your entries are neat, legible, and within the space provided.
- Do NOT staple the form
- USCIS uses special scanners to read your forms and documents. The scanners will not properly read the information that is greyed out, highlighted, or corrected using correction fluid or tape.
- If you make an error, start over with a clean form. RETYPE.
- Complete the entire form.
- We prefer that you download forms from our website, complete them electronically, and then print your forms to submit. However, you can fill it with your hand in a black ballpoint pen and use Capital letters.
- Make sure you put your name on each page of the i-693 on the top right-hand side.
Submit Required Documents and Evidence:
- Submit the documents or evidence listed in the form instructions.
- Supporting documents must be in English or accompanied by an English translation.
- Submit copies unless original documents are requested. If you send an original document with your form, it will become part of the record and will not be returned to you automatically.
- If you have any attachments, make sure each attached page has your name and either your A-Number (if available).
Check Your Application:
To ensure your form is accepted for processing, make sure your name, date of birth, contact info, Alien Number (A number) passport, or driver's license number is correct.
- Sign the i-693 form in front of your Civil Surgeon who is doing your i-693 processing.
- Pay the correct fee. (as applicable). Otherwise, your application will be rejected. You may need a new i-693 sealed envelope.
- Answer all questions completely and accurately. Consult your attorney if needed.
- If filing multiple forms, write your name and date of birth EXACTLY the same way on each form.
- Mail the form(s) to the correct address using an approved method of delivery. Forms may be submitted using any form of USPS delivery or an approved courier service. Acceptable courier services include FedEx, DHL, UPS & USPS with tracking numbers. Always get proof of delivery/tracking number.
- If you have a RFE (Request For Evidence Letter from USCIS, enclose the RFE letter that you received with the sealed I-693 envelope given to you. Put a rubber band around it and put it in the courier service envelope.

Vaccine Waivers on Health Grounds
Immigration Applicants can be deemed ineligible if they lack proof of having had all required vaccinations. But USCIS may approve a waiver of vaccination inadmissibility based on health grounds. Civill Surgeon can grant Vaccine waivers based on your medical condition, pregnancy & contraindications. Please bring your health history with you for review by Civil Surgeon. There is an extra fee for providing assessment and waivers. You may also need a letter from your primary care physician who can substantiate your conditions for waiver.
Waiver (I-601) If the vaccination would go against the applicant's religious beliefs or moral grounds
The I-601 waiver, also known as the Application for Waiver of Grounds of Inadmissibility, is a form used by certain individuals who are not U.S. citizens who need to request a waiver for specific grounds of inadmissibility. These grounds of inadmissibility may include reasons such as vaccination waiver on religious grounds, unlawful presence, criminal offenses, fraud or misrepresentation, and more.
However, please note that our expertise is mainly on the USCIS i-693 form, essential for immigration medical exams. If you need more detailed information on the I-601 waiver, we recommend reaching out to an immigration attorney or an expert. We can work with legal experts & help you in filling the I-601 Form and presenting your case for waiver based on religious or moral grounds and other legal grounds as shown in form I-601.
While there are many grounds on which an applicant can be found inadmissible, under which USCIS or the Secretary of the DHS determines that requiring convictions will approve an I-601 waiver.
The current USCIS Filing Fee for the I-601 waiver is $1,050. The typical USCIS processing time for the I-601 waiver is 9 months to over 2 years. It is reported on the web that currently the approval rate is about 65%.
