Immigration Green Card Medicals
Form I-693, also known as the "Report of Medical Examination and Vaccination Record," is a form used by the United States Citizenship and Immigration Services (USCIS) to document the results of a medical examination for certain individuals applying for immigration benefits. The form is typically completed by a designated civil surgeon, such as Dr. Neema Malhotra, who is a doctor authorized by the USCIS to perform the medical examination.
The purpose of the medical examination is to screen for certain diseases that may pose a public health risk and to ensure that the individual is up-to-date on their vaccinations. The examination includes a physical examination, blood tests, and a review of the individual's vaccination records. The civil surgeon will then complete Form I-693 and provide it to the individual, who will then submit it as part of their immigration application.
The cost of the Immigration Medical Exam is $195 and filling out the I-693 form is $200.
We also do N-648 Waivers.
Annual, DOT, DMV, School, Sports, and Employment Physicals, Live Certificates & Preventive Health Panels are offered only at our San Jose location. USCIS Civil Surgeon Services, and Immigration Physicals are done at both Milpitas & San Jose locations.
