General FAQs
TeleMed Video Visits are NOT meant for emergencies. If urgent, dial 911 or go to the nearest emergency room or hospital.
Please review the list of conditions to see if your symptoms are suited for Tele-Medicine Virtual Visits. TeleMed visits are the best way next to clinic visits and consult your provider.
You can do your Immigration first visit via a virtual TeleMed Visit if you are not near our clinic & San Jose. If you are within 15 miles, it is preferred to come in person. But if you are coming from far away or have time constraints, we can do a video visit and expedite the process.
Here are the steps for a successful visit.
- Make a TeleMed Virtual appointment for the Immigration Medical Exam either online at our website at this link (Make Appointment)
- You will confirmation of the appointment and instructions. You will also get an invite link to join the video visit link from our EHR software.
- Make a payment of $395 per applicant that includes your medical exam & processing of the I-693 form by the Civil Surgeon. We will send you a payment link by email or text so that you can pay by credit card or HSA Card. You can also pay at our website https://www.uscisClinic.com/pay/. You will get an itemized receipt for reimbursement from your employer.
- Upon payment of $395, you will get a TeleMed Video link. Our staff (physician nurse or medical assistant) will review your vaccination and medical history to assess any gaps in vaccinations and determine all the labs needed for I-693 form processing. Make sure you have all available Vaccination records with details date the vaccination was given, by whom & vaccine manufacturer's name. Please have a state-approved photo ID such as a driver's license or a passport.
- After review of your medical history & vaccination records;
we will give you Lab Orders & Vaccination Orders. - You can take vaccinations from any pharmacy or healthcare provider. Insurance will cover your vaccination. If you do not have insurance, we will guide you to get Vaccinations from the cheapest place to get them.
- You can get labs done from anywhere. You will have a choice of pre-paid Lab orders from Quest or other labs at discounted volume prices. Only a few Insurances cover lab charges. You can pre-pay the labs take the pre-paid lab slip and walk into the lab with or without an appointment.
- Lab results take 3-4 working days. After your results come back & you have taken the vaccinations, you will make a follow-up appointment to see Civil Surgeon Dr. Neema Malhotra for your medical exam in person. You will sign your I-693 in the presence of the civil surgeon & show your photo ID.
- At the end of the visit, you will be given your Completed, signed & sealed envelope for submission to USCIS. You will be also provided an electronic copy of the I-693 form for your records.

Video Tele-Medicine or TeleMed Visits are not meant for emergencies. Call 911 if needed. Please review the list of conditions to see if your symptoms (as shown in the section below) are covered and can be addressed in virtual Tele-Med visits.
We use the HIPAA-compliant Tele-Med Virtual Visit application integrated with our EHR & Practice Management System to facilitate the workflow of our providers to serve you better.
You will either get an invite from us with a TeleMed meeting link or our medical assistant may start an instant video meeting and invite you to join. Please make sure you have done new patient registration and given your consent to see if you have paid for your visit or have made arrangements and have provided your insurance and credit card information.
After you have provided your insurance info, payment or credit card information, and given your email we will schedule a TeleHealth visit.
Self-pay and not insured or insured with high deductible plans are welcomed. Tele Med visits are affordable and have transparent pricing. We will send you a confirmed Tele-Med appointment notice; our application will send you via an email and or SMS notification 15 minutes before the scheduled meeting time. If this TeleMed consult is for the I-693 Immigration Medical Exam review of your medical history; make sure you have vaccination history readily available for review by our medical assistants or providers. This virtual visit is only to review your vaccination and medical history and give you orders for the Lab Tests and Vaccinations needed to be specific.
Per USCIS guidelines, you are required to appear later in person before the civil surgeon Dr. Malhotra and get a full medical examination done and show your passport or driver's license or a state-approved photo ID and sign the I-693 form before her.

The Tele-Med app provides the best experience on iPhone, a smartphone, a tablet, iPad, or Galaxy but you can also use your computer as long you have a webcam or camera and audio is enabled. We can use either Zoom or Google Meet or Office Ally applications.
Start Your Visit
Click on the link sent to you by our healthcare provider and start the video conference & join the meeting with your provider. Make sure you share your Video and Audio; otherwise, our provider will not be able to see you or hear you. Please join 10 minutes before and wait for your provider to join. If you have any difficulty joining the Tele-Med consult or your provider has not joined on time, please feel free to call us at 408-684-8600 to verify your appointment and or when the provider will be able to join you.

Test your device in your web browser ahead of the scheduled visit.
TIPS:
Have Adequate Lighting
Make sure you are sitting in a lighted environment so that the doctor can see your face and body well.
Have a Flash Light with you.
The doctor may ask you to zoom the flashlight and your camera at certain parts of your face or body.
Have your chief complaint, the reason for the visit, medical history, and questions or concerns you may have for the doctor.
If you need privacy with our provider, make sure you are in a place that can facilitate it.
The doctor Will Need Your Medical History
Please take your temperature, height, weight & pulse ahead of time for the exam.
If it is for your Immigration Medical Exam, make sure you have all the vaccination history available. If you do not have one, do not worry. We have various options available.
Be prepared to share your medical history, chief complaint, your weight, height, etc., and may be asked to zoom in with a camera and if needed with a flashlight to look at part of the body in affected areas. Make sure you have privacy. Your home is your exam room.
Make a list and be prepared to ask your questions and make sure your concerns have been addressed.
After the Visit
The doctor will send your Prescriptions to your preferred pharmacy or orders for Lab or Radiology and set up a follow-up visit to discuss the outcome of the results and plan treatment based on your results.

Payments are due prior to joining the physician for your TeleMed Virtual Visit. If you have insurance that we accept, you must first do New Patient Registration and provide a copy of your insurance card. We will check your eligibility, benefits, annual deductibles and visit co-payment. Please pay your deductibles are your share of the co-pay prior to joining TeleMed Video Chat with a physician.
You can pay at his link either by your Credit Card or by PayPal.
Please make sure you have done new patient registration and given your consent to see you have paid for your visit or made arrangements and have provided your insurance and credit card information. You will then either get an invite from us with a video TeleMed meeting link or our medical assistant may start an instant video meeting and invite you to join.
Make sure your Insurance Info (patient name and date of birth, insured name, subscriber or member number, date of birth, and address are provided ahead of time for the Video Visit so that we can check your eligibility and benefits ahead of time. It can avoid any misunderstanding about insurance coverage.
A deposit of $200 may be required for the first visit. It will be refunded after the insurance has been paid after deducting your copayment, coinsurance, deductibles & uncovered charges for the visit.
Immigration Medical Exams & labs are not covered by insurance. However, your employer may reimburse the adjustment of visa-related expenses. Clients will be required to pay $395 for the immigration medical exam before their video visit by the doctor.
You will get an invite and link to join the meeting or the doctor or medical assistant may start an instant Video Meeting with you. Click on the link sent to you. You may have to install an app before joining a visit. Check your camera & audio are working. You may join with your computer, laptop, or IOS devices such as iPhone, iPad, or Android devices.
