We do physicals & medical exams for Schools, Colleges, Sports, DOT (Department of Transportation), DMV, Employment, and Care Facilities such as Day Care, Healthcare, Senior Care Facilities, and Immigration Physical Medical Exams for Green Card applicants. These different types of physical examinations are not typical annual physicals but these physicals serve different & specific purposes and requirements of various entities. We also do regular preventive annual physicals as part of annual preventive care for adults and children.

It's important to note that the specific requirements and procedures for these different types of physicals may vary depending on your medical history, the entity or agency, or the employer that requires the medical exam, the state laws, and the county and country you are in.

Please bring the Forms that you need to get filled out and instructions for your healthcare provider so that they can be filled out properly to assure your success. These exams are not considered annual physicals or checkups and are not usually covered by insurance, but some employers will reimburse you for the expenses, which can be paid by an HSA account. Rest assured, we only order lab tests or x-ray, or vaccinations only if needed for compliance and you do not have proof. We keep your cost to a minimum by sending you to the lowest-cost labs or pharmacy of your choice for vaccinations (if needed). We can refer you to a contracted lab for the lowest cost and expedient service.

DOT & DMV Physicals

DOT physicals are required for commercial drivers to ensure they meet the federal standards for physical qualifications as outlined by the Federal Motor Carrier Safety Administration (FMCSA). These physicals are more extensive than regular physicals and include vision and hearing tests, urine tests, and a thorough examination of the driver's medical history.

If you are applying for DMV Renewal or FMCSA or DOT Medical Exam, MCSA-5875 & or MCSA 5876, we are the go-to clinic for expedient & cost-effective medical exams. Same-day appointments are available.

We will guide you for the minimum cost & coordinate benefits with your insurance. The typical medical exam fee is $225

Care Facilities Physicals

Care facility exams are required for the staff seeking or continuing employment in Day Care Centers, Senior Care, Nursing Homes, Healthcare, Long-term Care Facilities, and Assisted Living Facilities. The typical cost of reviewing and filling up the form is $195. Please bring with you the form your employer wants you to get filled out by a physician. If any Lab Tests are required, we can send you to outside labs for the lowest cost.

Pre-Emploment Physicals

Pre-employment physicals are required for certain jobs, such as those in the healthcare, construction, and transportation industries, to ensure that employees are physically capable of performing the job duties safely. These physicals may include a thorough physical examination, drug testing, and screenings for specific conditions or diseases. The cost for the pre-employment exam is $195 to $275 depending upon the scope. Any labs or vaccinations will be extra.

Sport & School Physicals

School, college, and sports physicals are required for students participating in extracurricular activities to ensure that they are physically capable of participating and to identify any potential health concerns. These physicals typically include a thorough physical examination, height, weight, and vision checks, and may include screenings for certain conditions such as asthma or heart problems. The typical cost of sports physical and form filling is $195

Annual Physicals

Annual physicals are regular check-ups that are recommended for adults to maintain their overall health and to detect any potential health issues early on. They typically include a thorough physical examination, blood work, and screenings for conditions such as cancer, diabetes, and high blood pressure. In adults, usually one Annual Physical per year is covered by insurance as part of preventive.

If you are self-pay or without insurance, the cost of an annual physical check-up is $225, and a follow-up visit is $150 to review the lab results, and treatment plan and do any referrals or give a prescription. If labs and radiology are needed, we will send you to the lowest-cost labs, radiology, and continuity of care.

Live Certificate

A live certificate for a person refers to a document that confirms the existence and identity of an individual. This type of certificate is commonly used for background checks, visa or passport applications, or in some cases, for opening a bank account or collecting a pension. Examples of live certificates for a person include pension benefits eligibility or birth certificates, passport copies, and government-issued ID cards.

A live certificate is a certificate of proof of existence that confirms that a particular person is currently alive and exists. A Physician may be allowed to authenticate that the person is alive.

Please bring with you your specific form & instructions. Make sure to bring proof of identity such as a Passport, Driver's License, or Birth certificate. The cost of the Live certificate is $150

Immigration Physicals

Form I-693, also known as the "Report of Medical Examination and Vaccination Record," is a form used by the United States Citizenship and Immigration Services (USCIS) to document the results of a medical examination for certain individuals applying for immigration benefits. The form is typically completed by a designated civil surgeon, such as Dr. Neema Malhotra, who is a doctor authorized by the USCIS to perform the medical examination.

The purpose of the medical examination is to screen for certain diseases that may pose a public health risk and to ensure that the individual is up-to-date on their vaccinations. The examination includes a physical examination, blood tests, and a review of the individual's vaccination records. The civil surgeon will then complete Form I-693 and provide it to the individual, who will then submit it as part of their immigration application.

The cost of the Immigration Medical Exam is $200 and filling out the I-693 form is $200.
