HOW LONG THE MEDICAL EXAM PROCESS TAKES?

• It will require 2 visits. The average time for the expedited process is 4 to 7 working days, depending upon your vaccination, lab results, medical history, and your timely follow-up. If you have an urgency, we will help you expedite it for you.

• On 1st Visit:  a) Bring the i-693 filled form, vaccination records, Photo ID & medical history for our review. You will have a Physical Exam & our Physician will evaluate what Vaccines & tests you will need.
• We will give you order forms for Lab tests for Gonorrhea, Syphilis & TB (QuantiFeron Blood Test). Lab results can take up to 4 or 5 working days. Let us know when your Lab results come; we will make you an appointment for your second visit.
• After reviewing your medical history & age, we will determine the needed vaccines or give you orders for you to get them from anywhere you so desire. Vaccinations are covered by insurance as preventive care. We have Walgreens and CVS within a few blocks.

On Your 2^nd Visit:   Our USCIS Civil Surgeon will review your i-693 completed package, vaccines, and lab results and see if anything else is required per USCIS guidelines based on your info. You can download your Lab Results from the patient portal of the lab or our patient portal. You can download instructions on how to interpret your Lab Results by clicking here. 

• You will be given a sealed envelope of your exam to be submitted to USCIS or your attorney. A duplicate electronic PDF copy will be provided to you via your email.
• Rarely, do you need an extra  day for an X-ray of the chest, if your TB Test comes back positive.

If you are in a hurry, just call us and we may be able to expedite it by pre-ordering labs determining vaccination gaps in a TeleMed video consult, and doing your exam in one visit.

We do every effort to save you money and coordinate your insurance benefits for the Lowest Total Cost of your I-693 Medical Exam completion. All insurance will cover Vaccinations under preventive care.

Some insurance policies may cover Labs. If needed, you can get vaccinations from any pharmacy or any place and labs from anywhere for the lowest cost, convenience, and fast turnaround. We accept vaccination records from other countries. We coordinate benefits with your insurance and negotiate special rates with different labs & pharmacies to minimize your costs. During your first visit, we will give you all available options and guidance.

• Our Medical/Physical Exam Charges: $200
• I-693 Form Processing: The fee is $210 per form.
  Total Cost $410 includes two visits, visit for medical exam and 2nd visit to review your medical evaluations and fill I-693 form and process it.
  
  An electronic copy of the i-693 form processed by the civil surgeon is given free for your records. Insurances do not cover the cost of an Immigration medical exam. Your employer may pay for it. An immigration medical exam is different than your regular annual physical.
• 2nd I-693 Original Form copy is available at a discounted price of $195 if ordered at the same time as the first form.
• Vaccinations are covered by insurance with doctors' orders and may be taken at any pharmacy like CVS, Walgreens, Walmart, or Costco or from your primary care provider. If you do not have insurance, we will guide you for minimum cost and where to get vaccinations.
• Lab Tests: (Quantiferon, Gonorrhea & Syphilis) are required. We have compared all the labs in the Bay area. We will send you to the cheapest independent Labs such as Quest at 60% discounted prices. These labs have many locations including near our clinic in Milpitas & San Jose. For your convenience, a mobile Lab service can come to your home or work to pick up the samples. Our recommended lab costs 3 to 5 times less than other labs such as Lab Corp, Quest, Stanford, or Sutter. Some insurance may cover some of your labs. Lab charges can vary from $130 to $210 depending on your age & medical history & medical records.
• Your Employer may reimburse you for Medical Exam Related Expenses. Your HSA or FSA Card will also cover expenses. We will give you a receipt for your records & reimbursement from your employer.
• We accept all Credit Cards, HSA Cards, Cash, Checks, Zelle, Venmo, PayPal, Apple Pay, and Google Pay.

Cost of N-648 Disability Waiver:

Our charges for N-648 waivers are $795 per applicant.  This includes two visits with civil sureon and form completion. The initial consult only is $395 to review medical history and if you have compelling reasons for the waiver. This consult fee will be applied toward your N-648 form processing.

4. Payment by Credit Card, Cash, Check, PayPal, Apple Pay, or Zelle. HSA or FSA cards are accepted.

Form Filling Instructions:

One form per applicant is required. Type and Print only in black ink.

Fill the 1st page with Applicant info; Fill the first few lines on page 2 asking you how USCIS can contact you. Also, each page's top line has boxes asking for your family name, first name, and A-number. Please fill these boxes on all pages. If you do not have an A-number given; leave it blank.

DO NOT sign or put any date. You have to sign & date in presence of the Civil Surgeon.

After filling out the I-693 pdf form with your demographics; print the form single-sided. Do not use heavy paper as it is difficult to fold. Use 19-21 lb paper.  Ensure the bar code is printed on each page and your name is on each page. See the sample filled form and instructions.

What Vaccinations are required?

         Bring with you your Immunization Records to determine what will be needed per USCIS. Do not take any vaccine till you have consulted a Civil Surgeon such Dr. Neema Malhotra has reviewed your medical and vaccination history.

Effective 1/22/2025 USCIS has Waived COVID-19 Vaccination Requirement for Adjustment of Status Applicants (I-693).

a) Adults: Tdap, MMR, Varicella (Chicken Pox) & Influenza, for age > 2 years
b) Children: Must be up to date with a vaccination schedule for their age per CDC guidelines for Hepatitis A and B, Influenza and influenza b (Hib), Measles, Mumps, & Rubella (MMR), Meningococcal, Pneumococcal, Pertussis (whooping cough), Polio, Rotavirus, Tetanus & Diphtheria, Varicella (chicken pox). Vaccine schedules are available on our website and CDC websites. Effective June 2022, Hepatitis B is required for ages under 60.

Even if you do not have any vaccination records, you will be presented with all available options to meet your requirements for your health needs, cost, and expediency of filing the I693 form. You can consult your healthcare provider for your specific needs and applicable waivers per USCIS that may be granted by the Civil Surgeon.

For your convenience; we give you orders and you can take your vaccines at any pharmacy such as CVS, Walgreens, Costco, and Walmart near you. Vaccines are covered by insurance with doctor's orders. Just bring with you a receipt or proof of vaccination and we will record it in the I-693 medical exam form.

If you do not have immunization/vaccination records, read this section on this FAQ. You can either take an immunity test or take vaccines. You can develop immunity either through natural disease or through prior vaccination. In some cases, physicians or civil surgeon  can give selective waivers based on medical history. You can discuss it with us during the consult. 

Polio Virus Vaccine: is required for all ages.

COVID-19 vaccination is NOT required as of 1/22/2025.  View the current USCIS policy on vaccinations.

Note: If both MMR & VARICELLA are needed, both must be taken on the same day. You may take a combined MMRV Vaccine. Our staff will give you a guide on what, where & when to take vaccinations at no or low cost. 

If you are planning to take the vaccine, wear short sleeves or loose clothes so that your arms can be easily accessed by the healthcare provider for giving the vaccination.

You may elect to do Immunity Tests/Titers as you may have already immunity and need not take most of the vaccines. You can acquire immunity from disease in childhood or vaccination or both. The immunity tests are very affordable, Most of applicants opt to take immunity tests. You can discuss it with us, to determine the best option for you. 

What If I Do Not Have Vaccines Records

• Do not worry. Most of the applicants do not have all the vaccination records. You can do an inexpensive lab test for immunity showing that you have acquired immunity either via vaccination or natural disease. If you do not have immunity, then you can take vaccinations.
• Vaccines are required by USCIS for communicable diseases. Bring any records you may have. We can take any vaccine records that were administered by anyone including in a foreign country. The Physician will do their best within their discretion and USCIS guidelines based on your medical history; they give you choices to either do a test for immunity or give vaccines. You can decide based on your filing timeline, cost, and the health & safety of others.
• For Example; MMR requires two shots 28 days apart. We can get one today & schedule your take 2^nd shot for MMR later. You can file the application now & give the proof to your attorney or take it with you on your USCIS interview and or fill it with your application.

You are only required to receive a single dose of each vaccine when you visit the civil surgeon. You are encouraged to follow up with your private healthcare provider to complete the series. Once you have received the single dose appropriate at the time, the civil surgeon can sign and certify & seal Form I-693. The civil surgeon will indicate on the I-693 form that the next shots in the series have been scheduled.

Provide documentation of vaccination to the civil surgeon before completion of the medical examination.

Make sure you take the additional vaccination shots per schedule. It is good for your health and the health of the public. You may be asked during your interview with the USCIS field officer at the time of visa grant or approval to show proof of scheduled vaccination by the civil surgeon as indicated on your Green Card application.

Are Vaccines Safe? : Yes; Vaccines are safe.

• You can take another shot even if you have taken before.
• Consult your physician, if you are pregnant or sick or allergic to ingredients. You will need waiver from your physician or from Civil Surgeon for USCIS for these conditions. Speak to the physician or civil surgeon during your medical exam for guidance. 
• Civil Surgeon is also allowed to give vaccine waivers to applicants based on applicant's medical history, pegnancy, contraindications & religious beliefs.

     What Lab Tests are required?         

1. Quantiferon (Tuberculosis) for all ages over 6 months old.
2. Syphilis at or after age 18 and up.
3. Gonorrhea for all applicants between ages 18-24 years. If an applicant has an adverse sexual medical history, ages can be 15 and up.
   
4. For these lab tests, NO fasting is required.
5. General Lab guidelines are below. Some of the labs are required and others depend on your vaccination, medical history, and age. Please come and see Dr. Malhotra or the civil surgeon  first before you get the labs done.

   Lab Information

All your lab tests are done by an accredited outside major lab such as Quest, LabCorp, Foundation Lab or IHD Labs, etc.  They have multiple locations. You can take the pre-paid lab slips and walk in and no appointments or fasting is required. We have a volume group buying agreement with these labs to offer you discounted prices of up to 65% off. We have labs in our area and their charges range from $450 to $1,000 for the 3 lab-required tests but you can get a discount with our clinic orders. We will recommend you the cheapest Lab & cost-effective & convenient solution. We have these lab locations a mile or two away from our clinic and you can go to give your samples right after the medical exam in our clinic. We strive to give you convenience, cost-effective, and proven solutions and outcomes to get your green card without any delays. You can depend upon our experienced civil surgeon & staff.

Your insurance will not cover lab charges as these are very specific to immigration medical exams. Insurances cover labs (subject to plan deductibles) only if evidence of disease is present & you are visiting a physician for that reason. 

For convenience and the total low cost of your immigration medical exam; we have negotiated a package price for 3 tests for $190, if payable in advance at the time of the medical exam in our clinic. Your lab charges may range from $40 to $200 depending on your age & medical history. The independent labs are accepted by USCIS and are independent accredited facilities located across the Bay Area.

The lab locations are within two miles of both USCIS clinic's Milpitas & San Jose locations. No fasting or appointment is required. You can go to labs, as a walk-in w/o an appointment or go whenever to a convenient location near your home or work. 

Lab prices are subject to change by the lab. If you select the lab of your choice, you will be responsible for paying directly to the lab. We are not connected in any way to the labs. You are advised to check which labs are in your insurance network & if labs are covered under your insurance policy.

       The lab results take up to 3-4 working days which is the gating item in finishing your I-693 form & giving you a sealed copy for submission to USCIS. If you have deadlines, we can try to expedite the process to walk in or make same-day appointments & quick results.

Laboratory Testing: Effective 9/1/2025; Civil surgeons are permitted to order laboratory testing up to two weeks prior to the visit with the civil surgeon. The labs must be ordered by the civil surgeon, and the civil surgeon should be able to directly access the lab results. Hand-carried results cannot be accepted unless they can be independently verified with the laboratory.
We are connected with Quest, LabCorp, Foundation and IHD Labs to order and receive lab results, which are directly reported into our EHR.

To ensure chain of custody, prevent tampering, and maintain USCIS compliance, lab from Kaiser, Stanford, Sutter, or other labs must send results directly to the civil surgeon via fax, secure email, or postal mail. Hand-carried results cannot be accepted.

Is the Immigration Medical Exam the same as the Annual Physical?

The short answer is NO.

The purpose of the exam is to determine if the applicant is inadmissible to the United States due to a medical condition or a possible threat to society or public health. The Immigration Medical Exam differs from a general annual physical exam. Per USCIS guidelines, the medical exam focuses on communicable diseases, drug dependency, and other mental or physical conditions that can constitute an applicant to be a threat to society and or USCIS inadmissibility criteria. 

Immigration medical exam does not require or focus on your general preventive health tests, diseases such as lipids, kidney, lung, metabolic disorders, or heart or blood or bone diseases, etc. It is recommended that you book an annual physical with your physician or with Dr. Neema Malhotra where your annual physical which is usually covered by insurance as preventive. Immigration medical exam is usually not covered by insurance.

The USCIS Immigration Medical Exam is a medical examination required for certain United States immigrants. The exam is conducted by a USCIS-approved civil surgeon and includes a physical examination, a review of the applicant's medical history, and laboratory tests. You will be referred to one of civil surgeon in our practice network to complete your immigration exam and paperwork.

If you need an Annual Physical or a Sick visit, book another appointment. Those appointments are billable to insurance. Immigration medical exam are NOT billable to insurances. But preventive care such as vaccinations are covered by insurances.

What are the health-related grounds for inadmissibility or Green Card denial?

USCIS is not concerned about your personal health (such as heart disease, diabetes, obesity, lipid or renal disorders, cancer, etc.)  but how your disease, physical or mental disorders, or addictions affect public health & safety and if pose any danger to society.

If you have any prior citations or arrests or DUI, please inform the civil surgeon so that this issue can be mitigated. 

U.S. immigration law divides the health-related grounds of inadmissibility into the following four general categories:

A. Communicable diseases of public health significance;

B. Lack of proof of having received required vaccinations;

C. Physical or mental disorders with associated harmful behavior or a history of associated harmful behavior; and

D. Drug abuse or addiction.

See INA section 212(a)(1)(A). HHS regulations classify these and other medical conditions into class A or B medical conditions. Class A medical conditions result in inadmissibility while Class B medical conditions do not.

Civil Surgeon has to document the above in the I-693 Form for the Immigration Medical Exam.

Waivers for Vaccinations due to medical or religious reasons & Disability N-648 Waivers.

Civil Surgeon Dr. Neema Malhotra can review and do assessments, waivers, and certifications in case you have a history of citation of DUI or any substance abuse, criminal offense, or any other challenges. We can give Vaccination and X-ray waivers if you are pregnant or have an adverse medical history & or any contraindications.

We also do Medical exams and issue N-648 waivers for Green Card holders who are becoming a US citizens but are unable to read, write or speak English and pass the civic test due to disability or illness, or other challenges. Vaccine Waivers on Health Grounds

Immigration Applicants can be deemed ineligible if they lack proof of having had all required vaccinations. But USCIS may approve a waiver of vaccination inadmissibility based on health grounds. Civil Surgeon can grant Vaccine waivers based on your medical condition, pregnancy & contraindications. Please bring your health history for advise and review with you for review by Civil Surgeon. There is an extra fee for waivers.

Waiver (I-601) If the vaccination would go against the applicant's religious beliefs or moral grounds

The I-601 waiver, also known as the Application for Waiver of Grounds of Inadmissibility, is a form used by certain individuals who are not U.S. citizens who need to request a waiver for specific grounds of inadmissibility. These grounds of inadmissibility may include reasons such as vaccination waiver on religious grounds, unlawful presence, criminal offenses, fraud or misrepresentation, and more.

However, please note that our expertise is mainly on the USCIS i-693 form, essential for immigration medical exams. If you need more detailed information on the I-601 waiver, we recommend reaching out to an immigration attorney or an expert. We can work with legal experts & help you in filling the I-601 Form and presenting your case for waiver based on religious or moral grounds and other legal grounds as shown in form I-601.

While there are many grounds on which an applicant can be found inadmissible, under which USCIS or the Secretary of the DHS determines that requiring convictions will approve an I-601 waiver.

The current USCIS Filing Fee for the I-601 waiver is $1,050. The typical USCIS processing time for the I-601 waiver is 9 months to over 2 years. It is reported on the web that currently the approval rate is about 65%.

We charge an additional $200 to handle the I-601 waiver support documentation. There is NO guarantee that I-601 waivers will be granted by USCIS. It is the sole decision made by USCIS and or DHS.

You can do your Immigration first visit via a virtual TeleMed Visit.

Here are the steps.

1. Make a TeleMed Virtual appointment for the Immigration Medical Exam either online at our website at this link  (Make Appointment)
2. You will confirmation of the appointment and instructions.
3. Make a payment of $395 per applicant that includes your medical exam & processing of the I-693 form by the Civil Surgeon. We will send you a payment link by email or text so that you can pay by credit card or HSA Card. You can pay at our website at link https://www.uscisClinic.com/pay/. You will get an itemized receipt for reimbursement from your employer.
4. Upon payment of $410, you will get a TeleMed Video link. Our staff (physician nurse or medical assistant) will review your vaccination and medical history to assess any gaps in vaccinations and determine all the labs needed for I-693 form processing. Make sure you have all available Vaccination records with details date the vaccination was given, by whom & vaccine manufacturer's name. Please have a state-approved photo ID such as a driver's license or a passport.
5. After review of your medical history & vaccination records; we will give you Lab Orders, Vaccination Orders and X-ray orders.
6. You can take vaccinations from any pharmacy or healthcare provider. Insurance will cover your vaccination. If you do not have insurance, we will guide you to get Vaccinations from the cheapest place to get them.
7. You can get labs done from anywhere. You will have a choice of pre-paid Lab orders from Quest, LabCorp, Foundation or IHD labs at discounted volume prices. Only a few Insurances cover lab charges. Most insurances do not cover labs. You can pre-pay the labs and take the pre-paid lab slip and walk into the lab with or without an appointment.
8.  Lab results take 3-4 working days. After your results come back & you have taken the vaccinations, you will make a follow-up appointment to see Civil Surgeon for your medical exam in person. You will sign your I-693 in the presence of the civil surgeon & show your photo ID.
9. At the end of the visit, you will be given your Completed, signed & sealed envelope for submission to USCIS. You will be also provided an electronic copy of the I-693 form for your records.

How to Interpret Immigration Lab Results and Immunity Tests

Lab Results Interpretations.

A Sample Immigration Lab Report is shown below & how to interpret your lab results. 

 If results are within range and normal,  usually are shown in GREEN color, which means no further action is required. You can come and get your I-693 completed form. If the results are out of the normal range (abnormal), usually show in RED color, which means that further action is required. If QF TB Test is positive (Red color), you need an X-ray to rule out active TB. If immunity tests are abnormal ( Red Color), then you need to take that vaccination. If RPR or Gonorehea tests are reactive (abnormal) usually shown in RED color, just call us for the next steps to perform confirmatory tests and discuss options.

QF TB, Syphilis, & Gonorrhea Tests should be Non-Reactive or Negative is considered normal. If you are Reactive, you had an infection in the past. If the TB test is Positive must have a chest X-ray (2 views) to rule out active TB. Orders given to you at the time of first visit, also include X-Ray orders.

Immunity Tests for Hep-B Surface Antibody, MMR, Varicella, etc. should be Positive or Reactive, meaning you have acquired immunity either through taking vaccines or natural immunity by disease.  Non-Ractive or Negative results mean that you do not have immunity and need to take the vaccine. What If your QuantiFerron Lab TB Blood Test comes back positive?

TB is a contagious disease. It is part of the requirement by USCIS & CDC to make sure you do not have an active TB infection. Do not worry. 

About 10 % of the Asian, African & South American population comes with TB positive. Per USCIS, CDC & local public health requirements, the applicant must do an X-ray of the chest of two views of the lungs to determine if your TB is active or latent (dormant). Only about 1 to 2% of the X-rays show an active TB. You have about a 1 in 500 chance that you may have an active TB. Not to worry much. But if you have it, you need to get treated for your own health sake and the health of others. 

The X-ray report signed by a radiologist stating no evidence of TB is included with the I-693 form and sent to USCIS. In addition, civil surgeons have to also inform the local county public health department and send a copy of the TB Test and X-ray report. If TB lesions show an active TB, you will have to undergo treatment & the local county public health department is informed and they will monitor you & treat you. If TB lab test is positive and X-ray report shows no evidence of TB, tratment not mandatory but recommended. Consult your primary care own provider (PCP) for advise.

We have given you already an X-ray order that will be required in case your TB Blood test is positive. You can take this order to any radiology facility. We have recommended a few places in the instructions, that are cost-effective (Cost less than $80 for 2 views of the chest) and send an X-ray report within a day or two.

2nd Visit Instructions to pick up I-693

To ensure a smooth experience and a successful & timely completion of your I-693 medical exam, you must read the following instructions: Read info, FAQs & instructions at uscisClinic.com.  

Download 2nd Visit Instructions to pick up your I-693 in Forms section in our website. Do not worry, we will give you a copy of these instructions and go over them with you during your first visit with us. 

After your initial review of each applicant vaccinations, You will be given three (3) ORDERS for
 1) Labs  2) Vaccination and  3) X-ray. Show the given ORDER to each facility as needed.
Get your Order slip back, you will need it later. If you are proactive, you can get i-693 processed in a week.

• LAB ORDERS Instructions for Applicant:
  Go to the Lab and give your samples.
            NO Fasting is required. Lab Results take 4 to 5 working days.
             If you pre-paid Lab orders, take the orders to either Quest, LabCorp, Foundation or IHD labs.
             We have given you the Quest, LabCorp, Founation & IHD Lab location map, hours & how to make an appointment & view your results. You can go to any one of these labs with prepaid orders.
  • Quest: Make appointments online: https://appointment.questdiagnostics.com . Select test type> “All Other Tests”. Sign up with Quest’s patient portal at QuestDiagnostics.com to view results.

  • LabCorp: Make appointments & View results online at https://labcorp.com/patients/

  • IHD accepts walk-ins. No appointment is needed. Ask IHD to consent to email you the lab results.
  • Foundation lab will email you labs if you elect to do so.

INTERPRETING LAB RESULTS

• For the QF TB, RPR Syphilis, and Gonorrhea Test, the results should be Negative or Non-Reactive.
• If the QF TB Test is positive, an X-ray is required to rule out Active TB (See X-Ray Orders). If TB blood test is positive, you will need a follow up visit at extra cost for TB counselling and morbidity report is filed with public health departments. 
• Immunity Tests for MMR, Varicella, and HEP B: If IMMUNE, POSITIVE, or REACTIVE or the results values indicate that you have immunity, no Vaccinations are needed. If values are below an immunity level, that means you have not acquired immunity. You will need the vaccination for that disease.
  In Quest Reports: Normal results within range limits are in GREEN color and is good and No action is needed. RED color indicates abnormal results. TB Positive or Red color means you will need an X-ray.
  IHD Reports: Disregard any colors. Look for Results values. If TB Results are positive or out of range, you need an X-ray. If Immunity Test results show low or no immunity value, you need a shot.

        If you prefer to use your own Lab provider (Kaiser, Stanford, PAMF, etc.); you will need to get our orders approved by your insurance or provider. Schedule an appointment with your Lab and bring us the printed Lab results on their letterhead. DO NOT show or print and give us us results on your phones. Your performing lab must send the results directly to civil surgeon or our clinic to maintain chain of custody. You will be responsible for all your Lab costs, timeliness & the accuracy of your results.

• VACCINATION ORDER Instructions for Applicant:
  • Your Vaccination Orders will indicate what shots are needed. If it is in your orders for MMR, Varicella, and Hep-B, only if Lab results show no immunity.
  • You can get vaccinations from any pharmacy (Walgreens, Costco, Walmart, Safeway, or CVS) or your primary care provider. Provide your insurance information & our orders to the vaccination provider, who will bill your insurance. For more vaccination info visit https://usciscisclinic.com/uscis/vaccinations.
    If you do not have immunity to the disease, you will need to take the vaccine. In addition, take other vaccination included in your orders such as Influenza (Flu),Tdap, Pneumonia, etc. 
  • If you do not have insurance, no worry, we will guide you to get one at the cheapest or no cost.
• After your lab results have come ( 4-5 working days) and you have completed your vaccinations, then Book an appointment on our website for 2nd Visit to pick up the i-693 form sealed envelope.
  

3)  X-RAY ORDERS (ONLY IF YOUR TB TEST IS POSITIVE)

• If your TB blood test result is Positive, you need to undergo a Chest X-ray with two views to rule out active
  Having active TB is not common (only 1 in 500). X-ray costs range are $80 to $100. Check pricing & opening hours on X-ray facility's websites or search engines.

    Ask the X-ray facility to Fax results/report directly to our clinic's Fax number: (408) 650-7417 within 48 hours.

• You can take our Order slip to one of the following recommended X-ray (Radiology) or any other facility you like.
  • Valley Radiology/RadNet Imaging: in Santa Clara, Alameda, Contra Costa, San Francisco, Solano etc. counties.

  • Just-Xray: 244 N. Jackson Ave Suite #110. Website: com. Walk-ins are welcome. No appointment is needed. Tel: 408-272-2727.
  • If you are going to Kaiser, PAMF or any other facility for X-rays, you must also bring an X-ray report signed and dated by a radiologist on their letterhead or get it Faxed to (408) 650-7417. To comply with USCIS, the report must state the date the X-ray was taken, the date it was read, and clearly state "NO Evidence of active TB". This radiological report will be attached to your I-693 form for USCIS and submitted to the local health department. X-ray films are NOT needed for submission.

        I-693 FORM instructions

• If you have not already given us your filled I-693 form, download the latest I-693 form from our website and fill in the first four sections with your demographics. Bring us i-693 with all 14 printed pages with barcodes. Do not sign or date You must sign it before the Civil Surgeon.
• Follow the instructions on our website. Right-click on the downloaded form and select Adobe Reader as the application to open it. Do not use your browser. Adobe Reader will auto-fill your name on the top of each of the fourteen pages. You can see detailed instructions & view a correctly filled sample I-693 form on uscisClinic.com in the Forms section.
• If you have difficulties printing the form, you can email us a filled i-693 PDF form. We can help you fill & print it for you at a nominal cost of $20.

       2nd VISIT PLANNING & BOOKING to pick up your I-693 sealed envelope:

• The 2nd Visit is to sign & pick up the completed I-693 form in a sealed envelope. After your Lab results are out and you have completed your vaccinations per our orders, Book an Appointment for your 2nd visit on our website at the same location as your 1st We are in Milpitas on Mon, Wed & Fri & in San Jose on Tue & Thu.
  • Please allow one hour for each applicant. During the 2^nd visit, our staff will process your I-693 form.
  • Bring printed Vaccination Records, Labs, & Reports of items listed in your order. Do not show it on your phone as this can result in errors on the form. Do NOT bring your whole medical history, just items in order.
  • Please also bring a valid Photo ID such as a current Driver's License or a Passport.
• We will review the completed I-693 filled form with you, provide you with an electronic copy, and seal the I-693 form in an envelope that you can send to your attorney or to USCIS directly via FedEx, UPS, or USPS etc.
• Civil Surgeon and their staff will fill in your I-693 form by entering the medical info, labs, vaccinations, & findings. After the applicant has done physicial exam and signed before civil surgeon, sign & scan it. You will be given your sealed I-693 Envelope for USCIS submission & an electronic copy for your records. Your I-693 forms must be picked up within 30 days of your 1st visit, otherwise, you may have to do some lab tests again & an additional $100 fee will apply for new orders & evaluation.
• Each applicant must appear once before the Civil Surgeon and sign the original I-693 form in her presence.
• If you have a Request for Evidence (RFE) from the USCIS; bring it with you as RFE that is usually included outside of the sealed envelope. Always send your I-693 completed forms with at Tracking number via FedEx, UPS, or USPS envelope.
•  I-693 form is valid till forms expiry date & may be submitted anytime. 60 days submission requirement has been now waived.

Time is of Essence.

 Your i-693 form signed & sealed in an envelope by the Civil Surgeon is usually valid for 2 years, until or unless asked by the USCIS otherwise.

You are expected to send the sealed envelope to the USCIS promptly. Your completed I-693 medical exam sealed envelope must be received by USCIS to process your application, otherwise may delay your approval of your green card application.

You must pick up your I-693 from our Clinic within 30 days of your first visit i.e. initial medical exam. Otherwise, the form will be discarded and a review of documents & new form-filling fee of $130 will apply.

If the delay is more than 30 days, you may have to repeat sexual diseases lab tests.

USCIS Policy Highlights effective 4/10/2024

Effective April 4, 2024, any Form I-693 that was properly completed and signed by a civil surgeon does not expire and can be used indefinitely as evidence to show that the applicant is not inadmissible on health-related grounds.

Newly I-693 Form sealed & certified by a Civil Surgeon can be used indefinitely & does not expire till form expiry date. You can expect a new form submission after 2 years.

    Below is the latest policy updated on April 10, 2024

• Clarifies that a properly completed Form I-693 signed by a civil surgeon before November 1, 2023, continues to retain its evidentiary value for 2 years from the date of the civil surgeon's signature or the form expiry date on top of the form.

• Reiterates that USCIS officers have the discretion to request further evidence or a new or updated Form I-693, even if the original Form I-693 submitted is otherwise valid, if they have reason to believe the Form I-693 submitted does not accurately reflect the applicant’s medical condition or a new form has been released.
