The I-693 Immigration process is rather simple.
On your first visit, bring your existing vaccination records, if you have them. If you do not have vaccination records, no worries. Download and bring with you also the 14-page printed I-693 printed form with your demographics info on pages 1 and 2. The last of the 10 pages will be filled by Civil Surgeon after your medical exam and labs and vaccinations (if needed) are completed. Lab results can take 3 to 5 business days.
Download the I-693 form from our website on right. It is prepopulated with our clinic info on page 4. Make sure the applicant's name is populated on top of each page and the barcode is printed on the bottom of each page of the I-693 form. If any difficulty, no worries, we can do it for you at our clinic for a nominal fee.
On 2nd Follow-up visit, usually, a week later, you will be given an I-693 completed form in a sealed envelope and for your records an electronic copy of the I-693 completed form.

i-693 Immigration Medical Exam Quick Summary
Book an Appointment online or call us at 408-684-8600. Same or next-day appointments are available. It takes an average of 3-7 working days to complete the I-693 Medical Exam process.
On your first visit, bring with you any existing Vaccination Records (no worry, if you do not have them), a driver's license or passport, and an I-693 printed Form. The I-693 form & instructions can be downloaded here. During your first visit, during a medical exam, we will review your medical & vaccination history, assess required labs & gaps in vaccinations, and lead you through the entire immigration medical examination process.
We charge $200 for the i-693 immigration medical exam & $210 for processing & preparation of the I-693 form. USCIS required Lab tests can cost in the range of $130 to $220 depending upon the applicant's age & medical history & vaccination records. We refer you to outside labs a mile or two away. Lab results take 4-5 working days.
Vaccinations are covered by your insurance. You can take vaccinations at any pharmacy of your choice and just bring proof. If you do not have insurance, no worry, we will guide you to the lowest-cost pharmacy or places.

Please Upload your Insurance Card (both sides). Just take a picture and send it to us or upload it. If you do not have insurance or have high deductible plans, do not worry, we have fixed $225 cost for the first visit and $150 for the follow-up visit. We will recommend you to the lowest-cost, Lab, Radiology, X-rays, and Pharmacy in Silicon Valley to keep your expenses to a minimum. We will give you orders & recommend you to places where you can get the lowest cost & convenient services.
We will confirm your request for an appointment. If you need a same-day appointment, call us at (408) 684-8600. We keep some slots open that are not visible on the web.
We do not provide any emergency services. If it is an emergency, call 911 or go to the nearest emergency room or urgent care.
If you want to transfer your medical records from another healthcare provider, just fill out the attached Request for Medical Records Transfer and send it to us or your existing provider.
