Our Easy Immigration Process
1: Visit in-person or video chat to get Orders
Make Appointment
Same-day/next-day appointments are usually available. Group family appointments OK.
Initial visit can be in-person or virtual. Bring or upload all vaccination history. We will review your documents & identify any missing vaccinations and lab work.
No vaccine records or not taken in right intervals. No worries. Immunity/titers tests can be ordered in same blood draw for required TB and Sexual disease screening.
Get your Orders for Labs, Vaccinations & X-ray
2. Go to a Lab and give your specimen
Where: Quest & LabCorp, Foundation or IHD Labs (walk-ins are ok). No fasting required.
Tests: TB (QuantiFERON), Syphilis (RPR), Gonorrhea. Normal Results should be Negative / Non-Reactive.
Immunity tests: MMR, Varicella, Hepatitis B are reactive. If immune, no vaccine needed. If results below threshold, vaccination required.
Results: 3–6 business days. Pre-paid order results reported directly on our system. You can see lab results in lab patient portal.
3. Get Vaccinations & X-ray
Vaccinations: Get vaccinations per your Orders. It is only needed if labs show low or no immunity. You can take all vaccines on the same day. MMR and Varicella if ordered, must be taken on same day.
All Insurances cover vaccines at no cost under preventive care. Without insurance, go to budget pharmacies: Cure-All, Calaveras Pharmacy, Q-Bit, Costco, Walmart.
If TB test is Positive: Chest X-ray required.
4. Vist Civil Surgeon - Get Sealed I-693 Same Day
Once labs, vaccinations, and X-ray report (if needed) are all done, send all PDF copies of reports & your I-693 to us for our and civil surgeon’s review. If all looks ok, we will schedule your appointment to see one of our affiliated civil surgeons.
ALL applicants must appear in person for the second visit (first visit can be virtual, second cannot). See one of our civil surgeons closest to you and they will do physical exams and give you completed sealed envelope.
A civil surgeon performs the physical exam. You sign the I-693 in presence of civil surgeon and get your
completed i-693 sealed envelope and an electronic copy.

STEP 1: Day 1 >Make Appointment today & Bring Vaccinations records for review and Get Orders for Lab, Vaccinations and X-ray
Make an Appointment and come to our Milpitas or San Jose locations. You can make group appointments for your family. Same-day or next-day appointments are usually available.

Here is detailed guidance for your 1st Visit.
Bring any existing Vaccination Records. Do not worry, if you do not have vaccination records.
Download the latest I-693 Form (ver 1/20/25, expiring 9/30/27) & type in it or fill in your demographic info on the first 2 pages in Black ballpoint pen. One form for each applicant is required. Print all 14 pages single-sided. Read FAQs, cost, and form-filling instructions. Do not sign or date your I-693. You need to sign it in the presence of the civil surgeon.
Get your Immigration Medical Exam done. We will assess the Labs required & gaps in Vaccinations. You will be given orders for Labs, Vaccinations & chest X-rays if needed. We have a volume discount negotiated with the labs to save you money.

Step 2> Day 1: Go to Lab to give Sample
Take our Prepaid cost savings Lab Order and go to either LabCorp, Quest, Foundation Lab or IHD Lab locations near you or us. No fasting is required. We will give you the lab's location maps. Alternatively, you may arrange your lab yourself & take unpaid Lab orders, and pay the lab directly. Lab results take usually 4 to 6 working days. Results are reported to us directly in our elctronic records and you wll have access also.
At the lab's portals, you can make Appointment and View results online. For Quest at MyQuestDiagnostics.com , LabCorp at labcorp.com/patients , Foundation Lab at foundationlaboratory.com and IHD at Ihdlab.com/tests-for-individuals. Or call us or emails us if you need assistance. Most of locations allow walk-ins with our pre-paid orders. These labs give about 50% discount to our clients with pre-paid orders. You can interpret your lab reports as follows:
For the QF TB Test, RPR Syphilis Test, and Gonorrhea Test, the results should be Negative or Non-Reactive (GREEN Color)
If the QF TB Blood Test is Positive (RED Color), an X-ray is required to rule out Active TB. Refer to the X-Ray Orders section for recommended action & X-ray facilities. TB Test comes positive in about 10% of population except persons born Europe.
For immunity to tests; results values will show if you are immune or not and will require vaccination if no or low immunity.
STEP 3, Day 2> Get Vaccinations
Get your missing Vaccinations as per the order given on 1st visit and or if you do not have immunity per lab results. You can take all the vaccines on the same day. Our Civil Surgeon & staff will guide you on what, when & where you can get your vaccinations with or without insurance at no or low cost.
Go to any Pharmacy (Walgreens, Costco, Walmart, Safeway, CVS, etc.) or your provider of your choice and just bring proof of vaccination with you on your 2nd visit.
Insurances cover Vaccination costs under preventive care. If you do not have insurance, we will guide you during your visit to get vaccinations at low or no cost.
If multiple doses of a vaccine are required at certain intervals, You do not have to wait to complete the series. Just take the first shot of the vaccine and your I-693 will be processed with a note of insufficient interval indicating that additional shots have been scheduled by the applicant.
STEP 4: (Last Step) Day 5 : >See Civil Surgeon and Pick up your I-693 Form

e Make a follow-up 2nd appointment online. Allow about one hour for each applicant for your 2nd visit. See detailed 2nd visit instructions.
On the day of the appointment, bring your printed I-693 form with your demographiics per instructions in case you did not bring it on the 1st visit, and your driver's license or passport or state photo ID. Do not sign your I-693, you have to do it before the civil surgeon. The download links for forms and instructions are given in the section below on this page. The latest I-693 Form (ver 1/20/25 expiring 9/30/2027) can be downloaded from our website.
f you selected pre-paid orders, lab reports come to us directly in EHR. Print & bring your Vaccination records. Do not show us your reports on your phones. If you did not select the prepaid lab option, have your lab directly fax the reports to us to assure chain of custody.

One of our USCIS Clinic Civil Surgeon fill out your I-693 form with your Lab, Vaccination & X-ray reports. You will sign the form in the presence of Civil Surgeon & show your photo ID and a physical exam will be conducted by the Civil Surgeon.
You will be given your sealed I-693 Envelope for USCIS submission & an electronic copy for your records. Your i-693 form processsed by us expires on expiry date specified on I-693 form or as may deem necessary per USCIS.

- Labs at 50% discount with our Orders. Required Labs cost $130 to $200 based on age. Goto any Quest, Lab Corp or IHD with multiple locations in our bay area. We will help you find and schedule lab test near you.
FREE Vaccinations
- Vaccines are FREE and are covered by insurance under preventive care. If No insurance, we will guide you to lowest or zero cost. We don't charge you for vaccines. You can get them at any pharmacy for free with your insurance.

We have transparent pricing & charge $200 for the Medical Exam, and $210 for filling out the 14-page I-693 form by our experienced civil surgeon. The $410 cost includes both 1st & 2nd visits. We accept cash, all credit cards, HSA/FSA Cards, and checks. Expenses are reimbursable by employers if a visa is sponsored by an employer.
Vaccinations are covered by your insurance & can be taken at any pharmacy or provider of your choice. For self-paying applicants, we will guide you for the lowest or no cost vaccination providers.

1. It takes 2 visits: To complete the i-693 Medical Exam, Lab Tests, Vaccinations, etc., you'll need to make 2 visits and about 5 to 6 working days. Lab testing typically take 4 to 5 working days. However, if time is a factor, we can expedite the process and get everything done sooner and sometimes even in one visit. A clear and concise summary of the key steps is below to help guide you through the process.
2. During your First visit; an I-693 immigration-focused medical exam will be done. Our provider will take your medical history, assess any required vaccination gaps and give you lab & vaccination orders. You can take these orders to any place or a place referred by us to get vaccinations or labs done.
If you do not have vaccination records, do not worry. Most of the applicants from other than European countries do not have records. You can get an immunity test showing immunity from natural diseases or vaccinations.
For your first visit, please bring the following: vaccination records, a photo ID such as a valid Driver's License or a Passport & the all 14 pages of I-693 form filled with your applicant information on the first 2 pages and filled on top of each page with your name & A-number.
3. Download & Fill Your I-693 Form with your demographics: You are strongly advised to read about the I-693 medical exam & read FAQs & More Info on our website for detailed instructions. You can download the I-693 forms here.
PLEASE Download here the latest i-693 & instructions.
Brief Form Filling Instructions: You must fill the first and second pages of I-693 with your demographics and the top of each page with your name and A-number(if you have it). You can either fill it with BLACK ballpoint with Capital Letters or you can type into it using your computer. Use Adobe PDF Reader to fill and print the entire 14 pages of I-693 form for the Applicant Info section and print single-sided your filled I-693 without any printer scaling on regular weight (20 lb) paper. Our form is designed to be fillable and can pre-populate each page top line repetitive info of your Name and A-number provided you use Adobe PDF Reader and do not use browsers (Chrome or Edge or Safari) as the browsers extensions do not support auto filling I-693 form and printing barcodes.
A typed-in form is preferred over the hand-filled page and OCR scans better by UCIS for expedient processing. Make sure the barcode is printed on all pages. If you fill i-693 by hand, fill it with BLACK ink ballpoint pen and use CAPITAL letters. Do NOT sign or date your form. It has to be done before USCIS Civil Surgeon. 5. Appointments: You can also book your appointments online on our website 24x7 or call us at (408) 684-8600 during clinic hours.
6. Sealed Completed I-693 Form: After your Labs come back, at the 2nd appointment, you will get your sealed I-693 form envelope reviewed & signed by Civil Surgeon. You will also get an electronic copy of the completed I-693 for your records. Please do not leave without it.
You must pick up your I-693 within 30 days of the medical exam and submit it to USCIS or your attorney. Make sure to mail it via FedEx, UPS, or USPS with a tracking number.
7. Cost of Medical Exam:
We charge $200 for an immigration medical exam & $210 for filling out the I-693 form. The $410 price per person includes your 1st visit for a medical exam & 2nd visit to pick up the completed I-693 form.
Vaccinations are usually covered by insurance, while labs are not. We coordinate benefits with your insurance. We have negotiated special rates with different labs to minimize costs & pass on savings to you. Our clinic is happy to help you with any questions, concerns, or worries you may have, and our experience and expertise will ensure a successful, expedient & low total cost of the medical exam for processing with the USCIS processing of your Green Card. You can also get a second original copy of the i-693 sealed envelope at a lower cost.
Vaccinations are FREE if you have insurance & you can take vaccinations at any pharmacy near you such as CVS, Walgreen, Walmart, or Costco. Lab tests are done by an outside lab. We have negotiated special volume rates with different labs including Quest & LabCorp at a fraction of regular cost and they charge in the range of $40 to $200 depending on your age & medical history. We will guide you to the lowest-cost labs, and vaccinations & coordinate benefits saving you money. Our experience & expertise will ensure expedient processing and success with the USCIS processing of your Green Card while keeping your cost low.

The immigration medical exam is a crucial part of the green card application process. Under U.S. immigration law, all green card applicants must complete the immigration medical exam to confirm that they are in good health and are fit to live in the United States as permanent residents.
The examination must be done by a doctor who is designated by U.S. Citizenship and Immigration Services (USCIS). USCIS designates certain doctors (known as Civil Surgeons) to perform the immigration medical examination required for most green card applicants. An immigration medical exam is not the same as your routine annual physical medical. USCIS is less concerned about your personal health indicators such as cholesterol, blood chemistry, diabetes, heart health, etc., but more concerned about public health and inadmissibility.
In the immigration medical exam, you can expect:
- A review of your medical history and immunization (vaccine) records
- A physical and mental evaluation
- Drug and alcohol screening
- Tests for various diseases and illnesses (sometimes including an X-ray).
One of our affiliated Civil Surgeon will fill out Form I-693, Report of Immigration Medical Examination and Vaccination Record, and give you a sealed envelope with the form inside. You should not open this envelope. You should submit it along with your Form I-485, Application to Register Permanent Residence or Adjust Status, to USCIS.

- Medical/Physical Exam: $200 per Exam. (Valid for 2 years)
- I-693 Processing Fee: $210 per Form. Insurances do not cover this. Your employer may pay for it. An electronic copy of the i-693 form processed by the civil surgeon is given free for your records. The $410 price per applicant includes your 1st visit for a medical exam & 2nd visit to pick up the completed I-693 form. Your i-693 form processsed by us never expires.
- 2nd I-693 Original Form copy is available at a discounted price.
- Vaccinations are covered by insurance with doctor's orders and may be taken at any pharmacy like CVS, Walgreens, Walmart, or Costco, or from your primary care provider. If you do not have insurance, we will guide you for minimum cost and where to get vaccinations.
- Lab Tests: (QuantiFERON, Gonorrhea & Syphilis) are required by USCIS. We have compared all the labs in the Bay Area. We will send you to the cheapest independent Labs having many locations. We have negotiated volume pricing with three leading Labs & four radiology to save you money. Lab Tests range from $40 to $200 depending on age & medical history.
- Your Employer may pay for Medical Exam Related Expenses or use your HSA or FSA Card to cover your medical expenses.
We will give you a receipt for reimbursement from your employer or HSA account.
- We accept all Credit Cards, HSA Cards, Cash, Checks, Zelle, Venmo, PayPal, Apple Pay, and Google Pay.
