2. Go to a Lab and give your specimen<\/h3>\n\t\t\t\t\t\t

Where: <\/strong>Quest & LabCorp, Foundation or IHD Labs (walk-ins are ok). No fasting<\/strong> required.Tests: <\/strong>TB (QuantiFERON), Syphilis (RPR), Gonorrhea. Normal Results should be Negative<\/strong> \/ Non-Reactive<\/strong>.Immunity tests: <\/strong>MMR, Varicella, Hepatitis B are reactive<\/strong>. If immune, no vaccine needed. If results below threshold, vaccination required.Results: <\/strong>3-6 business days. Pre-paid order results reported directly on our system. 3. Get Vaccinations & X-ray<\/h3>\n\t\t\t\t\t\t

Vaccinations: <\/strong>Get vaccinations per your Orders. It is only needed if labs show low or no immunity. You can take all vaccines on the same day. MMR and Varicella if ordered, must be taken on same <\/strong>day.All Insurances <\/strong>cover vaccines at no cost under preventive care. Without insurance, go to budget pharmacies: Cure-All, Calaveras Pharmacy, Q-Bit, Costco, Walmart. \u00a0\u00a0\u00a0\u00a0<\/strong>If TB test is Positive: C<\/strong>hest X-ray required.\u00a0<\/p>\n\t\t\t\t\t\t\n\t\t\t\t\"uscis\n\t\t\t\t\t\t

4. Vist Civil Surgeon – Get Sealed I-693 Same Day<\/h3>\n\t\t\t\t\t\t

Once labs, vaccinations, and X-ray report (if needed) are all done, send all PDF copies of reports & your I-693<\/strong> to us for our and civil surgeon’s review. If all looks ok, we will schedule your appointment to see one of our affiliated civil surgeons.ALL applicants must appear in person <\/strong>for the second visit (first visit can be virtual, second cannot). See one of our civil surgeons closest to you and they will do physical exams and give you completed sealed envelope.A civil surgeon performs the physical exam. You sign the I-693 in presence of civil surgeon and get your
completed i-693 sealed envelope and an electronic copy.<\/strong><\/p>\n\t\t\t\t\t\t\n\t

Your initial visit can be in-person in our clinic locations or via a virtual video chat. One adult applicant in the family need to see us to go over vaccinations, medical history that you will provide. We explain you the process and assess gaps and give you orders for labs, vaccinations and X-ray. However, in second or final visit all applicants must appear before one of our Civil Surgeons team for physical exam, sign I-693 in civil surgeon presence and pick up your completed i-693 sealed envelope for USCIS submission.\u00a0<\/p>\n

One adult applicant in the family need to see us to go over vaccinations, medical history and explain you process and give you orders for labs, vaccinations and X-ray. However, in second or final visit all applicants must appear before one of our Civil Surgeons team for physical exam, sign I-693 in civil surgeon presence and pick up your completed i-693 sealed envelope.\u00a0<\/p>\n

Here is detailed guidance for your 1st Visit.<\/strong><\/a><\/p>\n
\n<\/strong>Bring any existing Vaccination Records. Do not worry, if you do not have vaccination records.\u00a0
\nDownload the latest I-693<\/a> <\/strong>Form (ver 1\/20\/25, expiring 9\/30\/27) & type in it or fill in your demographic info on the first 2 pages in Black<\/strong> ballpoint pen. One form for each applicant is required.\u00a0Print all 14 pages single-sided.<\/strong> Read \u00a0FAQs<\/a>, cost, and form-filling instructions.<\/a> Do not sign or date your I-693. You need to sign it in the presence of the civil surgeon.

\nGet your Immigration Medical Exam done. We will assess the Labs<\/a> required & gaps in Vaccinations<\/a>. You will be given orders for Labs, Vaccinations & chest X-rays if needed. We have a volume discount negotiated with the labs to save you money.\u00a0\n

Step 2> Day 1: Go to Lab to give Sample<\/h3>\n\"quest\n\t

Take our Prepaid cost savings Lab Order and go to either LabCorp, Quest, Foundation Lab or IHD Lab locations near you or us.\u00a0No fasting<\/strong>\u00a0is required. We will give you the lab’s location maps. Alternatively,\u00a0 you may arrange your lab yourself & take unpaid Lab orders, and pay the lab directly. Lab results take usually\u00a04 to 6<\/strong>\u00a0working days. Results are reported to us directly in our elctronic records and you wll have access also.<\/p>\n

At the lab’s portals, you can make\u00a0Appointment and View results\u00a0<\/strong>online. Most of locations allow walk-ins with our pre-paid orders. These labs give about 50% discount to our clients with pre-paid orders. Call or email us for any assistance.<\/p>\n

You can\u00a0interpret<\/strong>\u00a0<\/a>\u00a0your lab reports as follows:<\/p>\n

For the QF TB Test, RPR Syphilis Test, and Gonorrhea Test, the results\u00a0should be Negative or Non-Reactive<\/strong>\u00a0(GREEN Color)<\/p>\n

If the QF TB Blood Test is Positive (RED Color), an X-ray is required to rule out Active TB. Refer to the X-Ray Orders section for recommended action & X-ray facilities.\u00a0<\/strong>TB Test comes positive\u00a0in about 10% of population except persons born Europe.<\/p>\n

For immunity to tests; results values will show if you are immune or not and will require vaccination if no or low immunity.<\/p>\n

STEP 3, Day 2> Get Vaccinations<\/h3>\n\"vaccination<\/a>\n\t

Get your missing\u00a0Vaccinations<\/strong><\/a>\u00a0as per the order given on 1st visit and or if you do not have immunity per lab results. You can take all the vaccines on the same day. Our Civil Surgeon & staff will guide you on what, when & where you can get your vaccinations with or without insurance at no or low cost.<\/p>\n

Go to any Pharmacy (Walgreens, Costco, Walmart, Safeway, CVS, etc.) or your provider of your choice and just bring proof of vaccination with you on your 2nd visit.<\/p>\n

Insurances cover Vaccination costs under preventive care. If you do not have insurance, we will guide you during your visit to get vaccinations at low or no cost.<\/p>\n

If multiple doses of a vaccine are required at certain intervals, You do not have to wait to complete the series. Just take the first shot of the vaccine and your I-693 will be processed with a note of insufficient interval indicating that additional shots have been scheduled by the applicant.<\/p>\n

STEP 4: (Last Step) Day 5 : >See Civil Surgeon and Pick up your I-693 Form<\/h3>\n

e <\/strong>Make a follow-up 2nd appointment<\/a>\u00a0online. Allow about one hour for each applicant for your 2nd visit. See detailed\u00a02nd visit instructions<\/a>.<\/p>\n

On the day of the appointment, bring your printed\u00a0I-693 form<\/a> with your demographiics per instructions in case you did not bring it on the 1st visit, and your driver’s license or passport or state photo ID. Do not sign your I-693, you have to do it before the civil surgeon. The download links for forms and instructions are given in the section below on this page. The latest I-693 Form\u00a0<\/strong>(ver 1\/20\/25 expiring 9\/30\/2027)\u00a0can be downloaded from our website.<\/strong><\/p>\n

f you selected pre-paid orders, lab reports come to us directly in EHR. Print & bring your Vaccination records. Do not show us your reports on your phones. If you did not select the prepaid lab option, have your lab directly fax the reports to us to assure chain of custody.<\/p>\n

One of our USCIS Clinic Civil Surgeon fill out your I-693 form with your Lab, Vaccination & X-ray reports. You will sign the form in the presence of Civil Surgeon & show your photo ID and a physical exam will be conducted by the Civil Surgeon.<\/p>\n

You will be given your sealed I-693 Envelope for USCIS submission & an electronic copy for your records. Your i-693 form processsed by us expires on expiry date specified on I-693 form or as may deem necessary per USCIS.<\/p>\n\t\t\t\t\t
