What Civil Surgeon Does in I-693 Medical Exam?

Form I-693 is a mandatory medical examination required for immigration purposes in the United States. Civil surgeons are authorized by the U.S. Citizenship and Immigration Services (USCIS) to conduct these examinations. Here is the technical instructions summary for civil surgeons to conduct the I-693 immigration medical exam:

1. Obtain Form I-693 filled by the applicants with their demographic information. An interpreter's information is required if the applicant cannot understand English.
2. Verify the applicant's identity: Check the applicant's identification documents to ensure that they match the information on Form I-693.
3. Conduct a medical examination: Conduct a thorough medical examination of the applicant per USCIS guidelines, which includes:

• A complete medical history
• A physical examination
• A tuberculin blood test (or chest x-ray for those who have a positive test)
• Blood tests for syphilis & gonorrhea urine test for (if over 18 years old)
• Vaccination assessment and update, if necessary
• Assess any applicant's inadmissibility per USCIS guidelines.
• Refer to the public health department or specialists, in case a communicable disease is detected.

4. Complete Form I-693: Fill out the form completely and accurately, ensuring that all required fields are completed. Include your professional medical opinion on the applicant's health status and any recommended treatment.
5. Seal Form I-693 in an envelope: Once the examination is complete and the form is filled out, place the form in a sealed envelope with your signature across the seal. The applicant or anyone else should not open the envelope. (Only USCIS should open the envelope)
6. Give the sealed envelope to the applicant to submit along with their immigration application to USCIS.
7. Medical record: Keep a copy of the completed Form I-693 for the Civil Surgeon's records and also give a copy of the same to the applicant.

Your First Visit

Any Vaccination Records. Do not worry, if you do not have any records.

1. Valid Govt. Photo ids such as Passport or a USA Driver's License.
2. I-693 Printed Form, a 14 pages form expiring ver 03/09/23 expiring 03/31/25. Applicant must fill first 4 sections about demographics etc. The rest of the sections of the i-693 form will be filled by Civil Surgeon Doctor Malhotra.

Form Filling Instructions:

One form per applicant is required. Type and Print only in black ink.

Fill the 1st page with Applicant info; Fill a few lines on page 2 asking you how USCIS can contact you. Also, each page's top line has boxes asking for your family name, first name, and A-number. Please fill these boxes on all pages. If you do not have an A-number given; leave it blank.

DO NOT sign or put any date. You have to sign & date before Civil Surgeon Dr. Neema Malhotra.

After filling out the I-693 pdf form with your demographics; print the form single-sided. Do not use heavy paper as it is difficult to fold. Use 19-21 lb paper.  Make sure the bar code is printed on each page and your name is also on each page. See the sample filled form and instructions.

If you save it in a separate folder and open the form only with Adobe PDF Reader or Acrobat (not with Chrome, Edge or Safari, or other browsers); the top line on each page will fill automatically with your name & A-number as you type in the first page. Our civil surgeon Dr. Malhotra and clinic location info will also be printed. Please print it single page on regular weight (19-20lb) plain paper and not heavy or thick paper as scanners get jammed with heavy weight and are difficult to fold. Do not staple the I-693 Form. Do NOT sign the form.

Tip:  Using Browsers (Chrome or Edge or other viewers) to open I-693 may not fill i-693 properly print barcodes or scale it properly. Use only Adobe PDF Reader to fill out the I-693 form. Do not use browsers. While printing; use an ORIGINAL print setting on your printer i.e. without any print scaling or fitting the form to the page. HOW LONG THE MEDICAL EXAM PROCESS TAKES?

• It will require 2 visits. The average time for the expedited process is 4-6 working days, depending upon your vaccination, lab results, medical history, and your timely follow-up, and how well you filled your form & brought supporting documentation.
  
  On Your First Visit:
  a) Bring the i-693 filled form, Vaccinations records, Photo ID & medical history for our review.
  You will have Physical Exam & our Physician will evaluate what Vaccines & tests you will need.
• We will give you order forms for Lab tests for Gonorrhea, Syphilis & TB. Lab results can take up to 3-5 working days. Let us know when your Lab results come; we will make you an appointment for 2^nd
• We will give you needed vaccines or give you orders for you to get them from any pharmacy or facility or provider if you so desire.

On Your Second Visit :
             Our USCIS Civil Surgeon Dr. Neema Malhotra will review your i-693 completed package, vaccines, and lab results and see if anything else is required per USCIS guidelines based on your info.

• You will be given a sealed envelope of your exam to be submitted to USCIS or your attorney. A duplicate electronic PDF copy will be provided to you via your email.
• Rarely, do you need an extra day for an X-Ray of the chest if your TB Test comes positive. We will recommend you to the lowest-cost X-ray facilities in our area giving you results within a day. 
• You must pick up your I-693 sealed envelope within 30 days of a medical exam. You must submit I-693 sealed envelope within 60 days of signing & sealing by the Civil Surgeon.

What Vaccinations are required?

         Bring with you your Immunization Records to determine what will be needed per USCIS.
            a) Adults: Tdap, MMR, Varicella (Chicken Pox) & Influenza, for age > 2 years
            b) Children: Must be up to date with a vaccination schedule for their age per CDC guidelines for
Hepatitis A and B, Influenza and influenza b (Hib), Measles, Mumps, & Rubella (MMR), Meningococcal, Pneumococcal, Pertussis (whooping cough), Polio, Rotavirus, Tetanus & Diphtheria, Varicella (chicken pox). Vaccine schedules are available on our website and CDC websites. Effective June 2022, Hepatitis B is required for ages under 60.

You can consult your healthcare provider for your specific needs and applicable waivers per USCIS that may be granted by the Civil Surgeon.

For your convenience and low cost; we give you orders and you can take your vaccines at any pharmacy such as CVS, Walgreen, Costco, and Walmart near you. Vaccines are covered by insurance with doctor's orders. Just bring with your receipt or proof of vaccination and we will record it in I-693 medical exam form.

Covid -19 Vaccine Requirements:

Per recently published CDC guidelines; Covid-19 Vaccine is required as of 10/1/2021. Green Card Applicants will be required to show proof of vaccination to the Civil Surgeon during their I-693 Medical Exam. Make sure to bring your COVID-19 vaccination record with you. The latest USCIS Vaccine guidelines as of August 12, 2021, are at the link below. Minimum two COVID-19 shots & one COVID-BIVALENT shot are required.

COVID-19 vaccination is mandatory for USCIS per the current CDC schedule for anyone over the age of 6 months. Per recent USCIS guidelines, COVID-19 vaccine is required as of 10/1/2021. Green Card Applicants will be required to show proof of vaccination to the Civil Surgeon during their I-693 Medical Exam. Proof of vaccination will be required for I-693 form processing. Two initial Covid-19 shots are required as a minimum. In addition, 1 or more doses of the updated (2023-2024 Formula)  shot is required. If you are planning to take the vaccine, wear short sleeves or loose clothes so that your arms can be easily accessed by the healthcare provider for giving the vaccination.

NOTE: You are only required to receive a single dose of each vaccine when you visit the civil surgeon. You are encouraged to follow up with your private health care provider to complete the series. Once you have received the single dose appropriate at the time, the civil surgeon can sign and certify the Form I-693. The COVID-19 vaccine is the exception to this requirement. You must complete the COVID-19 vaccine primary series (for example, 1 or 2 doses, depending on which vaccine, in adults; 2 or 3 doses, depending on age and which vaccine, in children).  and provide documentation of vaccination to the civil surgeon in person before the completion of the medical examination. The civil surgeon will indicate on the I-693 form that the next shots in the series have been scheduled. 

The COVID-19 vaccine is the exception to this requirement. You must complete the COVID-19 vaccine primary series (for example, 1 or 2 doses, depending on which vaccine, in adults; 2 or 3 doses, depending on age and which vaccine, in children) and provide documentation of vaccination to the civil surgeon in person before completion of the medical examination. Make sure you take the additional vaccination shots per schedule. It is good for your health and the health of the public. You may be asked during your interview with the USCIS field officer at the time of visa grant or approval to show proof of scheduled vaccination by the civil surgeon as indicated on your Green Card application.

What If I Do Not Have Vaccines Records?

 If you do not have vaccination records, do not worry. Most of the applicants do not have all the vaccination records. You can do an inexpensive lab test for immunity showing that you have acquired immunity either via vaccination or natural disease. If you do not have immunity, then you can take vaccinations.

• Vaccines are required by USCIS for communicable diseases. Bring any records you may have. We can take any vaccine records that were administered by anyone including in a foreign country.
  The Physician will do their best within their discretion and USCIS guidelines based on your medical history; they give you choices to either do a test for immunity or give vaccines. You can decide based on your filing timeline, cost, and the health & safety of others.
• For Example; MMR requires two shots 28 days apart. You can get one today & schedule your take 2^nd shot for MMR later. You can file the application now & give the proof to your attorney or take it with you on your USCIS interview and or fill it with your application.

Are Vaccines Safe? : Yes; Vaccines are safe.

• You can take another shot even if you have taken it before.
• Consult your physician, if you are pregnant or sick, or allergic to ingredients. You will need a waiver from your physician or Civil Surgeon for USCIS for these conditions. Speak to the civil surgeon during your medical exam for guidance. 
• Civil Surgeon is also allowed to give vaccine waivers to applicants based on the applicant's medical history, contraindications & religious beliefs.

In general, you can take any vaccine either 2 weeks before or 2 weeks after your Covid-19 vaccine injection. New CDC guidelines show that you can take any vaccine within a few days of taking the COVID-19 vaccine until there are some contraindications based on your health history as determined by your healthcare provider. If you have any concerns, ask our providers during your medical exam.

     What Lab Tests are required?         

1. Quantiferon (Tuberculosis) for all ages
2. Syphilis after age 15 and up.
3. Gonorrhea for all applicants between ages 18-24 years. If an applicant has an adverse sexual medical history, ages can be 15 and up.
   
4. For these lab tests, NO fasting is required.
   
   Lab Information

All your lab tests are done by an accredited outside lab such as Quest, Bio-Reference, etc. We will recommend you the cheapest Lab. There are 5 labs in our area and their charges range from $450 to $1,000 for the 3 lab-required tests. Your insurance will not cover lab charges as these are very specific to immigration medical exams. Insurances cover labs (subject to plan deductibles) only if evidence of disease is present & you are visiting a physician for that reason. 

For convenience and the total low cost of your immigration medical exam; we have negotiated a package price for 3 tests for $190, if payable in advance at the time of the medical exam in our clinic. Your lab charges may range from $40 to $190 depending on your age & medical history. The independent labs are accepted by USCIS and are independent accredited facilities located across the Bay Area.

The lab locations are within two miles of both USCIS clinic's Milpitas & San Jose locations. No fasting or appointment is required. You can go to labs, as a walk-in w/o an appointment or go whenever to a convenient location near your home or work. 

Lab prices are subject to change by the lab. If you select the lab of your choice, you will be responsible to pay directly to the lab. We are not connected in any way to the labs. You are advised to check which labs are in your insurance network & if labs are covered under your insurance policy.

       The lab results take up to 3-4 working days which is the gating item in finishing your I-693 form & giving you a sealed copy for submission to USCIS. If you are in a hurry, we can try to expedite the process to walk in or make same-day appointments.

Lab Results Interpretations.

QF TB, Syphilis, Gonorrhea, and  Test:  Should be Non-Reactive or Negative is considered normal. If you are reactive, you have an infection. If the TB test is positive must have a chest X-ray to rule out active TB.

Immunity Tests for Hep-B Surface Antibody, MMR, Varicella, etc. should be Positive or Reactive, meaning you have acquired immunity either thru taking vaccines or natural immunity by disease.  Non Reactive or Negative results mean that you do not have immunity and need to take the vaccine.

What If your QuantiFerron Lab TB Blood Test comes back positive?

TB is a contagious disease. It is part of the requirement by USCIS & CDC to make sure you do not have an active TB infection. Do not worry. 

About 10 % of the Asian, African & South American population comes with TB positive. Per USCIS, CDC & local public health requirements, the applicant must do an X-ray of the chest of two views of the lungs to determine if your TB is active or latent (dormant). Only about 1 to 2% of the X-rays show an active TB. You have about a 1 in 500 chance that you may have an active TB. Not to worry much. But if you have it, you need to get treated for your own health sake and the health of others. 

The X-ray report signed by a radiologist stating no evidence of TB is included with the I-693 form and sent to USCIS. In addition, civil surgeons have to also inform the local county public health department and send a copy of the TB Test and X-ray report.

If TB lesions show an active TB, you will have to undergo treatment & the local county public health department is informed and they will monitor you & treat you. 

We have given you already an X-ray order that will be required in case your TB Blood test is positive. You can take this order to any radiology facility. We have recommended a few places in the instructions, that are cost-effective (Cost less than $100 for 2 views of the chest) and send an X-ray report within a day or two.

  An electronic copy of the i-693 form processed by the civil surgeon is given free for your records. Insurances do not cover the cost of an Immigration medical exam. Your employer may pay for it. An immigration medical exam is different than your regular annual physical.
• 2nd I-693 Original Form copy is available at a discounted price if ordered at the same time as the first form.
• Vaccinations are covered by insurance with doctors' orders and may be taken at any pharmacy like CVS, Walgreens, Walmart, or Costco or from your primary care provider. If you do not have insurance, we will guide you for minimum cost and where to get vaccinations.
• Lab Tests: (Quantiferon, Gonorrhea & Syphilis) are required. We have compared all the labs in the Bay area. We will send you to the cheapest independent Labs having many locations. For your convenience or mobile Lab, service can come to your home or work to pick up the samples. Our recommended lab costs 3 to 5 times less than other labs such as Lab Corp, Quest, Stanford, or Sutter. Some insurance may cover some of your labs. Lab charges can vary from $40 to $200 depending on your age & medical history.
• Your Employer May Pay for Medical Exam Related Expenses. Your HSA or FSA Card will also cover expenses. We will give you a receipt for your records & reimbursement from your employer.
• We accept all Credit Cards, HSA & FSA Cards, Cash, Checks, Zelle, Venmo, PayPal, Apple Pay, and Google Pay.

Cost of N-648 Disability Waiver:

Our charges are $795 per applicant. The initial consult only is $200 to see if you could be eligible for such a waiver. This fee will be applied toward your N-648 form processing.

Is the Immigration Medical Exam the same as the Annual Physical?

The short answer is NO.

The purpose of the exam is to determine if the applicant is inadmissible to the United States due to a medical condition or a possible threat to society. The Immigration Medical Exam differs from a general annual physical exam. Per USCIS guidelines, the medical exam focuses on communicable diseases, drug dependency, and other mental or physical conditions that can constitute an applicant to be a threat to society and or USCIS inadmissibility criteria. 

Immigration medical exam does not require or focus on your general preventive health tests, diseases such as lipids, kidney, lung, metabolic disorders, or heart or blood or bone diseases, etc. It is recommended that you book an annual physical with your physician or with Dr. Malhotra where your annual physical which is usually covered by insurance as preventive. Immigration medical exam is usually not covered by insurance.

The USCIS Immigration Medical Exam is a medical examination that is required for certain immigrants to the United States. The exam is conducted by a USCIS-approved civil surgeon and includes a physical examination, a review of the applicant's medical history, and laboratory tests.

What are the health-related grounds of inadmissibility?

USCIS is not concerned about your personal health (such as heart disease, diabetes, obesity, lipid or renal disorders, cancer, etc.)  but how your disease, physical or mental disorders, or addictions affect public health & safety and if pose any danger to society.

If you have any prior citations or arrests or DUI, please inform the civil surgeon so that this issue can be mitigated. 

U.S. immigration law divides the health-related grounds of inadmissibility into the following four general categories:

A. Communicable diseases of public health significance;

B. Lack of proof of having received required vaccinations;

C. Physical or mental disorders with associated harmful behavior or a history of associated harmful behavior; and

D. Drug abuse or addiction.

See INA section 212(a)(1)(A). HHS regulations classify these and other medical conditions into class A or B medical conditions. Class A medical conditions result in inadmissibility while Class B medical conditions do not.

Civil Surgeon has to document the above in the I-693 Form for the Immigration Medical Exam.

We do accept the initial first-visit walk-in without appointments for I-693 applicants. Applicants with confirmed appointments are given priority. 

You can do your Immigration first visit via a virtual TeleMed Visit.

Here are the steps.

1. Make a TeleMed Virtual appointment for the Immigration Medical Exam either online at our website at this link  (Make Appointment)
2. You will confirmation of the appointment and instructions.
3. Make a payment of $395 per applicant that includes your medical exam & processing of the I-693 form by the Civil Surgeon. We will send you a payment link by email or text so that you can pay by credit card or HSA Card. You can pay at our website at link https://www.uscisClinic.com/pay/. You will get an itemized receipt for reimbursement from your employer.
4. Upon payment of $395, you will get a TeleMed Video link. Our staff (physician nurse or medical assistant) will review your vaccination and medical history to assess any gaps in vaccinations and determine all the labs needed for I-693 form processing. Make sure you have all available Vaccination records with details date the vaccination was given, by whom & vaccine manufacturer's name. Please have a state-approved photo ID such as a driver's license or a passport.
5. After review of your medical history & vaccination records;
   we will give you Lab Orders & Vaccination Orders.
6. You can take vaccinations from any pharmacy or healthcare provider. Insurance will cover your vaccination. If you do not have insurance, we will guide you to get Vaccinations from the cheapest place to get them.
7. You can get labs done from anywhere. You will have a choice of pre-paid Lab orders from Quest or other labs at discounted volume prices. Only a few Insurances cover lab charges.  You can pre-pay the labs and take the pre-paid lab slip and walk into the lab with or without an appointment.
8.  Lab results take 3-4 working days. After your results come back & you have taken the vaccinations, you will make a follow-up appointment to see Civil Surgeon Dr. Neema Malhotra for your medical exam in person. You will sign your I-693 in the presence of the civil surgeon & show your photo ID.
9. At the end of the visit, you will be given your Completed, signed & sealed envelope for submission to USCIS. You will be also provided an electronic copy of the I-693 form for your records.

• _{Our team has reviewed your medical history and vaccinations carefully & assessed the gaps per USCIS guidelines.}
• _{Each applicant has been provided with personalized ORDERS for the Labs, Vaccination, and x-ray based on their age, medical & vaccination history. Show the given PLAN/ORDER to each facility as needed. Get your Order slip back, as you may need it later.}

_{LAB ORDERS}

• _{Lab orders typically include tests for TB and sexual diseases. In addition, if you do not have vaccination records, you may be given an immunity test that can show if you have acquired immunity through natural disease or prior vaccinations.}
• _{If you choose to use our recommended Lab facilities (Quest Diagnostics, you can visit any of their convenient locations throughout California. We have provided you with a map of 18 Quest Lab locations near our area, many are near our clinics.}
• _{No fasting is required for all these immigration lab tests.}
• _{You can book an appointment at 1-877-277-8772 or visit https://appointment.questdiagnostics.com/ . You can also walk into the lab without an appointment, but there may be some waiting time. Prior appointments are preferred by Quest.}
• _{After making the payment at our clinic, you can proceed directly to the Quest lab anywhere.}
• _{The lab will draw your blood for TB, Syphilis, and immunity tests and, if ordered, collect your urine sample for a Gonorrhea test.}
• _{You can sign up with the Quest patient portal at QuestDiagnostics.com to access your results.}
• _{Lab results typically take 3 to 4 working days. If you have any concerns or questions about your results, please wait at least three working days after giving samples before contacting us.}

        _{Insurance Coverage for Labs:}

• _{Most insurance does not cover immigration or employment-related lab expenses. But your employer may cover it.}
• _{If you prefer to use your Lab provider (e.g., Kaiser, Stanford, PAMF, Lab Corp), you will need to get our orders approved for coverage from them. Schedule an appointment with your chosen Lab and bring printed Lab results to our clinic on their letterhead.}
• _{If you encounter any difficulties or delays in getting the labs done, we can electronically submit your pre-paid Lab Orders to Quest.}

_{INTERPRETING LAB RESULTS}

• _{For the QF TB Test, RPR Syphilis Test, and Gonorrhea Test, the results should be Negative or Non-Reactive.}
• _{If the QF TB Test is positive, an X-ray is required to rule out Active TB (refer to the X-Ray Orders section on the next page).}
• _{Immunity tests for MMR, Varicella, and HEP B:}
  • _{"IMMUNE," "POSITIVE," or "REACTIVE" results indicate acquired immunity through natural disease or prior vaccinations, and no further shots are needed.}

_VACCINATIONS

• _{Your Vaccination Orders indicate which shots are required based on your Lab results. Visit com/uscis/vaccinations/ for more info on vaccinations.}
• _{You can get vaccinations from any pharmacy (CVS, Walgreens, Costco, Walmart, Safeway) or your primary care provider.}
• _{Provide your insurance information & our orders to the vaccination provider, who will bill your insurance as preventive care.}

_{TIP: Bring printed Vaccination Records, Labs, and Reports. Do not show them on your smartphone, can result in errors on your I-693 form.}

 X-RAY ORDERS}

• _{If your TB blood test result is positive, you need to undergo a chest X-ray with two views to rule out active TB (with a 1 in 300 chance). The cost ranges between $60 to $100. Check the pricing and open hours of imaging centers on their respective websites or through Google.}
• _{The results of the X-ray will be available within 48 hours.}
• _{The imaging center will fax the results directly to our clinic's Fax number: (408) 650-0717.}
• _{You can take our order slip to any of the following X-ray (Radiology) clinics:}

  • _{If you are going to Kaiser or any other facility of your choice, you must bring an X-ray report signed and dated by a radiologist on their letterhead. To comply with USCIS, the report must state the date the X-ray was taken, the date it was read, and state "NO Evidence of TB". This radiological report will be included with your I-693 form and submitted to the local health department.
    X-ray films are NOT needed for submission.}

• _{If you have not already done so, and given us your I-693 form, download the I-693 form from our website and fill in the first four sections with your demographics. Do not sign or date the form; You will sign it before the civil surgeon Dr. Neema Malhotra}
• _{Follow the instructions on our website. Right-click on the downloaded form and choose Adobe Reader as the application to open it. Do not use your browser. Adobe Reader will auto-fill your name on the top of each of the fourteen pages.}

• _{If you have difficulties printing the form, you can upload or email us a filled PDF form, and we can print it for you for $20.}

       _{2nd VISIT PLANNING & BOOKING:}

• _{After your lab results have come back and you have completed your vaccinations per our orders, book an appointment for your 2nd visit on our website uscisClinic.com. This visit is for picking up your sealed envelope containing the completed I-693 form.}
• _{Please allow one hour for each applicant during the 2nd visit. Dr. Neema Malhotra and her staff will process your I-693 form by entering the medical information, labs, vaccinations, and findings. They will review the completed form with you, provide you with an electronic copy, and seal the I-693 form in an envelope.}
• _{After your lab results have come back and you have taken or have completed your vaccinations, then Book an appointment on our website com for 2nd Visit for your i-693 form sealed envelope pick up.}
• _{Allow one hour for each applicant for your 2nd visit, during which Dr. Neema Malhotra and her staff will process your I-693 form by entering the medical info, labs, vaccinations, and findings & review the completed I-693 form with you, send you an e-copy & seal the I-693 in an envelope. After the applicant has signed before Dr. Malhotra, she will review, process & sign it. You will be given your sealed I-693 Envelope for USCIS submission & an electronic copy for your records. Your medical exam is good for two years. I-693 forms must be picked up within 30 days of your 1st visit to our clinic and submit it to USCIS.}

• Be patient. I know this is easier said than done. You miss your family and you’re frustrated with the delays. We understand. However, getting angry won’t help your application go faster.
• Get your application in. Yes, the lines are long, but the sooner you get your application completed the sooner you are at least in line.
• Submit the best application with the strongest evidence possible. Do everything you can to make sure your application won’t be rejected. Recheck your application.
• Type or Print your application in black ink. Follow the instructions.

Time is of Essence.

Your i-693 form signed & sealed in an envelope by the Civil Surgeon is usually valid for 2 years, until or unless asked by the USCIS otherwise.

You are expected to send the sealed envelope to the USCIS promptly. Your completed I-693 medical exam sealed envelope must be received by USCIS to process your application, otherwise may delay your approval of your green card application.

You must pick up your I-693 from our Clinic within 30 days of your first visit i.e. initial medical exam. Otherwise, the form will be discarded and a review of documents & new form-filling fee of $100 will apply. If the delay is more than 30 days, you may have to repeat some sexual diseases lab tests.

Effective 4/10/2024, once the I-693 Form is signed & sealed by the Civil Surgeon, it is valid for submission indefinitely. It has no expiry date. 

USCIS Policy Highlights effective 4/10/2024

Newly I-693 Form sealed & certified by a Civil Surgeon can be used indefinitely & does not expire.

Effective April 4, 2024, any I-693 form that was properly completed and signed by a civil surgeon on or after Nov. 1, 2023, does not expire and can be used indefinitely as evidence to show that the applicant is not inadmissible on health-related grounds.

• Policy Clarifies that a properly completed Form I-693 signed by a civil surgeon on or after November 1, 2023, retains its evidentiary value and does not expire.

• Clarifies that a properly completed Form I-693 signed by a civil surgeon before November 1, 2023, continues to retain its evidentiary value for 2 years from the date of the civil surgeon's signature.

• Reiterates that USCIS officers have the discretion to request further evidence or a new or updated Form I-693, even if the original Form I-693 submitted is otherwise valid, if they have reason to believe the Form I-693 submitted does not accurately reflect the applicant’s medical condition.
